White line disease is progressive breakdown of the inner non-pigmented hoof wall at its junction with the sole. Keratin-degrading bacteria and fungi colonise separated horn, creating a dry chalky or crumbly cavity that can extend much farther upward than the small ground-surface defect suggests. The sensitive laminae are usually not inflamed until debris pressure, abscessation or major wall instability develops.
Early disease is often painless and discovered by a farrier during trimming. Tapping may reveal a hollow wall, and powdery horn can be scraped from the white line. Extensive separation removes mechanical support from a large portion of the hoof capsule, permitting pedal-bone displacement and lameness. One or several feet can be involved.
Hoof distortion, long toes, flares, cracks, chronic wet–dry cycling and previous laminitis can create access and leverage, but there is no single proven organism or management fault. White line disease differs from laminitis: it primarily degrades hoof horn from the distal margin, whereas laminitis damages living laminar attachment. The two can coexist, and radiographic pedal-bone displacement changes treatment.
Topical products cannot sterilise an enclosed cavity beneath undermined wall. Complete exposure and mechanical correction are the core principles. Prognosis is excellent when disease is removed before displacement, although full return waits for new wall to grow from the coronary band. Recurrence is common if hidden undermined horn, flare or moisture conditions remain.
White line disease is separation and opportunistic degradation of the inner hoof wall, usually beginning at the ground surface and tracking upward without primary infection of sensitive laminae. The outer wall can look intact while a hollow area undermines support. Rotation may occur mechanically in severe cases, but this is distinct from laminitis-induced separation.
Failure may remain painless until wall support is substantially compromised.
At each trim, the white line is inspected for widening, chalky horn, cavities and a hollow sound on gentle wall percussion. The farrier maps the apparent margins, but the distal opening often underestimates proximal spread. Pain, digital pulse or lameness suggests debris pressure, abscess or structural compromise rather than simple superficial colonisation.
Tangential and lateral radiographs outline gas-filled wall separation and show pedal-bone position, sole depth and any laminitic change. Radiopaque markers over the external wall improve interpretation. Multiple angles may be required to define the circumference. Culture rarely identifies a single causal organism and does not replace removal of diseased horn.
Differentials include laminar wedge after chronic laminitis, a draining abscess, keratoma and ordinary stretched white line. Feet are re-examined after each staged resection because seemingly solid margins can conceal further undermining. Horses with prior disease need closer farrier surveillance, particularly during wall flare or environmental change.
Percussion and radiographs define how far the hidden cavity extends beneath apparently sound wall.
All detached and undermined hoof wall is resected back to a firm, tightly attached margin, exposing the diseased cavity to air. This tissue is already non-supportive, but removal must avoid damage to sensitive laminae. Small lesions can be managed during routine farriery; large resections require veterinary radiographs and a stabilising shoe, plate or composite support that does not seal infection inside.
After exposure, the area is kept clean and dry and treated with an appropriate topical antiseptic protocol. Products cannot compensate for incomplete resection. Hoof balance, flares and long breakover are corrected gradually. When pedal-bone displacement is present, mechanics follow chronic-laminitis principles with sole and caudal support.
Work is limited according to wall removed and stability, not simply absence of lameness. The defect must grow down from the coronary band, which takes many months. Rechecks remove any recurring crumbly margin before it tracks proximally. Prognosis for full performance is good without pedal-bone displacement; even displaced cases can do better than severe rotational laminitis if support is restored. Failure usually reflects hidden diseased wall, premature sealing or uncorrected mechanical separation.
Wall reconstruction waits until diseased horn is removed and the remaining structure is dry and stable.
