West Nile virus encephalitis is a mosquito-borne infection that can inflame the brain and spinal cord of horses. Birds maintain and amplify the virus, and mosquitoes transmit it from infected birds to horses and people. Horses and humans are generally considered dead-end hosts because the amount of virus in their blood is too low to continue the usual mosquito cycle.
Most infected horses show no illness. Those that develop neurological disease may have fever, reduced appetite, depression, weakness, incoordination, stumbling and difficulty rising. Fine muscle twitching, especially around the muzzle and neck, altered sensitivity to touch, head pressing, aimless wandering, impaired vision, difficulty swallowing, recumbency and seizures can occur. Spinal signs and muscle fasciculations are often prominent, but no single pattern distinguishes West Nile from every other encephalitis.
Risk follows mosquito activity, bird infection and regional climate rather than direct exposure to a sick horse. A clinical case therefore does not usually require contagion quarantine, although vector control and diagnostic precautions remain important. Direct handling does not ordinarily transmit the virus; exposure to neural tissue during necropsy is a different hazard and requires professional protection.
Survival depends on neurological severity. Horses that remain standing have a better outlook than those with paralysis or prolonged recumbency. Recovery may take weeks to months, and residual weakness, gait abnormality or behavioural change can persist. Vaccination markedly lowers the risk and often reduces severity in breakthrough cases, but protection requires a completed primary course and boosters timed for the local mosquito season.
Mosquitoes acquire the virus from infected birds and transmit it to horses; affected horses are generally dead-end hosts and do not spread enough virus to infect another mosquito. Muscle fasciculations, weakness, ataxia, altered mentation and cranial-nerve deficits vary widely. Fever may be absent by the time neurological signs prompt examination, so season and mosquito exposure remain relevant.
There is no useful routine test for predicting which mosquito-exposed horse will become neurological. Owners should keep vaccination records current and investigate sudden weakness, stumbling, muscle twitching, altered awareness or inability to rise. Travel history, mosquito exposure, vaccination status and the timing of signs help define risk, but clinical appearance overlaps with many spinal and brain diseases.
Veterinary neurological examination localises and grades dysfunction. Detection of West Nile virus-specific IgM in serum is the principal evidence of recent infection; cerebrospinal fluid antibody can add support. Prior vaccination and cross-reactions affect interpretation of some antibody results. Routine blood tests are often nonspecific, while cerebrospinal fluid commonly shows inflammatory change. PCR of blood is usually insensitive because viraemia is brief and low by the time neurological signs develop.
Rabies, EHM, trauma, toxicities and other regional encephalitides must be considered. Suspect cases may be reportable depending on jurisdiction, and sampling follows local animal-health guidance. A horse that is falling, dysphagic or recumbent needs urgent assessment before transport because further injury and aspiration can occur.
No antiviral treatment has been proven to clear West Nile virus in horses. Care is supportive and matched to neurological deficits. Anti-inflammatory medication, intravenous fluids and nutritional support maintain comfort and hydration. Sedation or anticonvulsants may be needed for dangerous agitation or seizures, while dysphagic horses require protection from aspiration and an alternative feeding plan.
Deep bedding, limb protection, assisted rising and frequent repositioning help prevent injury in weak or recumbent horses. Sling support is useful only in selected horses that can bear some weight and tolerate it safely. Bladder function, manure output, pressure sores and respiratory status are monitored closely. Humane euthanasia is considered for irreversible respiratory failure, uncontrollable distress or recumbency that cannot be managed without suffering.
Recovered horses return to work slowly after coordination and strength stabilise. Physiotherapy may assist residual weakness, and owners should expect improvement over months rather than days. Prevention includes veterinarian-directed vaccination before mosquito season, eliminating standing water where practical, cleaning troughs, using approved repellents and stabling at peak mosquito times when local advice supports it. Treating a sick horse does not replace vaccination of unaffected horses, and antibiotics are reserved for secondary bacterial complications rather than the virus itself.
