Urticaria is sudden localised swelling within the superficial skin, producing soft raised wheals or plaques commonly called hives. Histamine and other mediators make small vessels leak fluid after an immune or non-immune trigger. Individual wheals often pit with pressure, merge into larger shapes and disappear within hours, sometimes reappearing elsewhere.
Potential triggers include medications, vaccines, insect bites, feed components, contact products, plants, infection, heat, cold, pressure and exercise. In many episodes no cause is identified. Most horses remain bright and comfortable, with little itching. Swelling involving eyelids, muzzle or deeper tissue is angioedema and deserves closer assessment.
Hives must be separated from anaphylaxis. Respiratory distress, weakness, pale gums, diarrhoea, collapse or rapidly progressive facial and throat swelling indicates a life-threatening systemic reaction, not a routine skin eruption. Firm nodules lasting days, crusting or hair loss also suggest another skin disorder rather than simple urticaria.
An isolated mild episode often resolves without consequence. Recurrent or chronic urticaria requires a structured exposure history instead of repeatedly attributing it to the most recent feed. Allergy tests can show sensitisation without proving clinical causation. Prognosis is excellent for acute hives and more variable when a trigger cannot be removed. The main medical risk comes from systemic allergy or unnecessary prolonged corticosteroid use, not from the appearance of uncomplicated wheals.
Wheals reflect superficial dermal oedema and usually move or disappear within a day, unlike fixed nodules or infection. Angioedema extends more deeply around the muzzle or eyelids. The visual severity of uncomplicated hives does not predict anaphylaxis; breathing difficulty, weakness, diarrhoea or circulatory collapse are the findings that signal systemic danger.
Repeated episodes can nevertheless cause enough swelling or irritation to disrupt normal use. Some recur unpredictably.
The veterinarian first assesses breathing, heart rate, mucous membranes, temperature, gastrointestinal signs and facial or throat swelling. Timing is reconstructed against every injection, medicine, supplement, feed batch, topical product, insect exposure and exercise during the preceding hours to days. Photographs are useful because wheals may vanish before examination.
Typical transient wheals are diagnosed clinically. Cytology or biopsy is reserved for lesions lasting more than about 24 hours in one place, painful nodules, crusting, vasculitis or treatment resistance. Blood tests investigate systemic illness when fever or other signs are present. Intradermal or serum allergy testing may guide recurrent-case investigation but cannot identify the cause without matching history and controlled avoidance or challenge.
Owners should mark or photograph several wheals and record how fast they move or resolve. Emergency signs trigger immediate care. A drug suspected of causing hives is recorded precisely and not re-administered casually; the veterinarian distinguishes the active ingredient from unrelated products before a permanent allergy label is applied.
A horse with anaphylaxis requires immediate veterinary epinephrine, airway and circulatory support; antihistamine alone is inadequate. For uncomplicated mild hives, removing a plausible trigger and observation may be sufficient because lesions often resolve spontaneously. Antihistamines have variable effectiveness in horses but can help selected cases.
Systemic corticosteroids rapidly suppress extensive or persistent urticaria, with dose and duration limited because of laminitis, infection and metabolic risks. A horse with insulin dysregulation needs particular caution. Cool water and protection from rubbing support comfort; harsh topical preparations add irritation without treating vascular leakage.
Recurrent disease is managed through one change at a time: stop nonessential supplements or topical products, control insects and document response before rechallenge. Suspected medicines are replaced under veterinary guidance. Chronic cases may need dermatologist-directed elimination trials, biopsy or allergen immunotherapy when a consistent hypersensitivity is demonstrated. Feed is not dangerously restricted and multiple ingredients are not removed at once without nutritional planning. The goal is trigger identification and the lowest necessary medication exposure, while every recurrence is rechecked for systemic signs rather than assumed benign.
Emergency instructions are written down because a future exposure can cause a more systemic response.
