Thrush is degeneration of frog horn with secondary microbial infection, producing a foul-smelling black discharge in the central or collateral grooves. Anaerobic organisms thrive where deep narrow sulci trap moist organic material and receive little oxygen. Hind feet are often affected, but any hoof with poor frog contact, contracted or sheared heels and limited movement can develop disease.
Early thrush may be noticed only during hoof picking. Advanced disease destroys horn, exposes sensitive tissue and causes pain or lameness, particularly when infection tracks deeply into the central sulcus. The frog can become ragged and undermined. A horse kept in a clean stable can still develop thrush when conformation and trimming create deep non-self-cleaning crevices; conversely, mud alone does not explain every case.
Thrush differs from canker. Canker produces proliferative, frond-like tissue that bleeds readily and often needs biopsy and aggressive debridement, whereas thrush primarily destroys horn and has characteristic malodour. Deep central-sulcus disease can also resemble heel dermatitis extending into the frog. Persistent one-foot infection may reflect a structural imbalance rather than inadequate topical product.
Prognosis is favourable when diseased horn is removed and the frog regains healthy load and ventilation. Applying caustic chemicals without correcting heel mechanics can burn living tissue while infection persists. Severe longstanding cases require months for normal frog horn to regrow and can recur if the central groove remains tightly contracted.
Thrush is degradation of frog tissue within an oxygen-poor, manure-contaminated central or collateral sulcus, often involving Fusobacterium and mixed organisms. A black malodorous discharge is typical, but deep central-sulcus disease can be painful without extensive surface debris. Contracted heels and poor frog contact perpetuate the environment even in a clean stable.
Lameness indicates deeper sensitive tissue involvement rather than a merely cosmetic frog odour.
The foot is cleaned and the frog, central sulcus, collateral grooves and heel bulbs inspected under good light. Black malodorous material and soft undermined horn usually make the diagnosis clinically. A hoof pick or blunt probe is used gently to assess depth and pain without driving contamination into tissue.
Hoof balance, heel contraction, sheared heels, frog ground contact and shoe design are evaluated by a veterinarian and farrier. Lameness, a strong digital pulse, deep tract or bleeding warrants closer exploration and sometimes radiographs to exclude abscess or deeper infection. Proliferative white tissue or exuberant bleeding suggests canker and may require biopsy.
Owners can monitor odour, discharge, sulcus depth and tenderness at each cleaning. Lack of clear improvement after debridement and dry management indicates an incorrect diagnosis, inadequate exposure of diseased horn or persistent mechanical cause. Routine bacterial culture is rarely helpful in simple thrush because mixed environmental organisms are expected, but it can assist complicated deep infection.
A hoof knife is used conservatively to expose pockets without removing healthy weight-bearing frog.
Loose necrotic frog is conservatively debrided by an experienced farrier or veterinarian so oxygen and topical medication reach the affected grooves without removing healthy load-bearing tissue. The hoof is picked daily and kept on clean, dry footing. An appropriate astringent or antimicrobial solution is applied to the cleaned sulci; strongly caustic agents are avoided on exposed sensitive tissue.
Corrective trimming and shoeing open contracted grooves, balance the heels and restore appropriate frog function. A bar shoe or pad may support selected feet after infection is controlled, but a sealed pad over active contamination can worsen the environment. Regular movement on dry safe footing improves normal expansion and self-cleaning when the horse is sound.
Painful deep disease may need protective bandaging and treatment of secondary cellulitis, but systemic antibiotics are not routine for superficial thrush. Canker or abscess receives its own treatment. Bedding and turnout are managed to reduce prolonged manure and urine contact, while recognising that mechanics must also change. Follow-up continues until solid healthy frog has filled the defect; stopping when odour first disappears allows recurrence in a deep central sulcus.
