Tetanus is a frequently fatal neurological disease caused by tetanospasmin, a toxin produced when Clostridium tetani spores germinate in damaged tissue with little oxygen. Spores are common in soil and manure. Puncture wounds, castration sites, foot injuries, surgical wounds and retained placental tissue can provide suitable conditions, yet the original wound may be tiny, healed or never found.
The toxin travels along nerves and blocks inhibitory signalling in the spinal cord and brainstem. This produces rigid rather than weak paralysis: a stiff gait, difficulty turning, erect ears, flared nostrils, a raised tail and a characteristic anxious expression. The third eyelid may sweep across the eye when the horse is startled. Jaw stiffness, difficulty swallowing and generalised spasms follow, with noise, light or touch triggering violent contractions. Severe cases cannot stand or breathe normally.
Horses are among the most susceptible domestic species. Tetanus is not transmitted from one horse to another, so isolation for contagion is unnecessary, although the wound itself requires hygienic care. Disease does not create reliable protective immunity; a horse that survives still needs vaccination.
Prognosis is guarded and worsens with rapid progression, inability to remain standing, severe spasms and respiratory compromise. Prevention is far more reliable than treatment. Correct primary vaccination and boosters, plus additional wound-related protection when indicated, are essential even for horses that rarely leave the property. Waiting for stiffness after a puncture wound means toxin may already be irreversibly bound to nerve endings.
Clostridium tetani spores germinate in low-oxygen wounds and release toxin that blocks inhibitory nerve signals. The original wound may be tiny, healed or hidden in a hoof, reproductive tract or surgical site. Rigid gait, third-eyelid prolapse, lockjaw and stimulus-triggered spasms reflect toxin already bound to nerves; the horse remains conscious and frightened during severe episodes.
Tetanus is primarily a clinical diagnosis. Any unvaccinated or overdue horse with progressive stiffness, exaggerated responses, third-eyelid prolapse or difficulty chewing requires emergency examination. The history should cover wounds, injections, surgery, castration, foaling and hoof abscesses during the preceding weeks, but absence of a visible injury does not lower concern enough to wait.
The veterinarian observes gait, jaw and neck movement, tail tone, swallowing and stimulus-triggered spasms while minimising disturbance. Blood tests are nonspecific and mainly assess complications. Culture of a wound is often negative and does not rule out disease. Poisoning, meningitis, severe muscle disease, hypocalcaemia and other causes of rigidity are considered, but treatment should begin on strong clinical suspicion.
Preventive review confirms dates and product type for the primary vaccine series and boosters. After a high-risk wound, immune status determines whether toxoid booster, short-term antitoxin or both are indicated. The wound must be examined even when the horse appears normal, because early cleaning removes the environment in which bacteria can produce toxin.
Treatment aims to neutralise toxin not yet bound to nerves, eliminate further bacterial production and keep the horse alive while affected nerve endings recover. Tetanus antitoxin is administered promptly using a veterinarian-selected route and dose. The wound is located where possible, opened, debrided and drained, and appropriate antimicrobials are given. Antitoxin cannot remove toxin already fixed at the neuromuscular system, so early treatment matters.
The horse is nursed in a quiet, dark stall with minimal sudden sound or touch. Sedatives, muscle relaxants and other drugs reduce spasms; feeding and water are positioned to avoid unnecessary head lowering, with alternative nutritional and fluid support if swallowing is unsafe. Deep bedding, a sling in carefully selected cases, catheter care and frequent repositioning help prevent injuries in weak or recumbent horses. Airway support or mechanical ventilation may be required but is not widely available for adult horses.
Vaccination with toxoid begins or is boosted during recovery because illness itself does not confer dependable immunity. Nursing often continues for weeks as new nerve connections form. Humane euthanasia should be considered when uncontrollable spasms, prolonged recumbency or respiratory failure make recovery unrealistic. Wound hygiene and complete vaccination of other horses are reviewed, but contacts do not require quarantine because tetanus is not contagious.
