Sweet itch, also called Queensland itch or Culicoides hypersensitivity, is an allergic skin reaction to proteins in the saliva of biting midges. Sensitised horses mount an exaggerated immune response to bites that other horses tolerate. Disease is seasonal where midges are active but can persist much of the year in warm wet climates.
Intense itching leads to rubbing of the mane, tail head, withers, back, belly midline and sometimes face or ears, depending on local midge species. Hair breaks, skin becomes excoriated and crusted, and chronic rubbing produces thickened folded skin. Secondary bacterial infection and self-trauma add pain. Horses can damage fences, rugs and themselves while seeking relief.
Culicoides breed in damp soil, marsh edges and decaying organic matter and often feed around dawn and dusk in low wind. Susceptibility varies strongly between individuals and breeds. The condition is not contagious, although several allergic horses on one property can flare during the same insect season. Onchocerciasis, pinworms, lice, mites, dermatophytes and other allergies can mimic parts of the distribution.
There is no simple cure that removes sensitisation. Prevention must begin before seasonal itching becomes severe, because established inflammation and rubbing are harder to control. Prognosis for comfort is good with rigorous exposure reduction, but management is labour-intensive and relapse is expected when barriers fail. Moving to a different microclimate can dramatically help some horses, yet it is not practical or uniformly effective.
Culicoides midge saliva triggers a seasonal hypersensitivity, usually at the mane, tail head and ventral midline. Intense rubbing produces broken hairs, excoriations and thickened skin even though only tiny numbers of insects may be present. Midges fly most around dawn and dusk in sheltered humid conditions, so ordinary fly control may miss the relevant exposure.
Diagnosis uses the seasonal pattern, characteristic distribution, insect environment and exclusion of parasites and infection. The veterinarian examines mane and tail hair breakage, belly lesions, chronic skin thickening and secondary pyoderma. Skin scrapings, coat brushings, tape preparations or fungal testing investigate lice, mites and ringworm; perianal tape testing helps exclude pinworms when tail rubbing dominates.
Intradermal or serum allergy tests can support identification of insect sensitisation but have false positives and negatives and should not replace the clinical pattern. Skin biopsy shows hypersensitivity but is not specific to Culicoides. Treatment response to well-executed midge avoidance adds evidence.
Owners can record weekly itch scores and lesion photographs against weather, dawn and dusk turnout and product use. This distinguishes true prevention from seasonal natural improvement. Itching that continues through insect-free months, marked weight loss or predominantly neurological rubbing warrants a broader investigation. Secondary infection is sampled before antibiotics when possible.

Exposure control is the foundation. A fine-mesh sweet-itch rug covers mane, tail and belly before the insect season; fit and heat stress are checked daily. Horses are stabled behind fine screens during local peak feeding times, often dusk and dawn, and fans create airflow that weak-flying midges avoid. Turnout is moved away from wet breeding sites where possible.
Approved repellents are applied at the correct concentration and interval, recognising that sweat and rain shorten protection. Environmental insecticides are used safely and breeding habitat reduced where feasible. Corticosteroids can rapidly suppress severe allergy but require veterinary dosing because repeated use carries laminitis and immune risks. Antihistamines have variable effect. Soothing topical therapy and treatment of bacterial infection allow damaged skin to heal.
Allergen-specific immunotherapy can be considered after specialist assessment, but response is inconsistent and slow. Mane and tail rubbing must be made physically safer while control develops. The following season’s prevention starts before insects emerge, not after raw skin returns. No feed supplement reliably substitutes for bite avoidance. Long-term success is judged by reduced itch and intact skin with the lowest necessary anti-inflammatory exposure.
