A subsolar hoof abscess is a focal pocket of infection and pus trapped between sensitive, living tissues and the rigid sole or hoof wall. Bacteria enter through tiny white-line separations, sole bruises, nail penetration, cracks or defects produced by wet–dry environmental change. Pressure builds in a space that cannot expand, explaining why a small abscess can cause dramatic pain.
The usual presentation is sudden severe lameness in one limb, sometimes almost non-weight-bearing. The foot can feel warmer with a bounding digital pulse, and hoof testers often identify a focal painful area. Infection may track under the sole or upward inside the wall and break out at the heel bulbs or coronary band. Once drainage occurs, comfort often improves rapidly.
Not every acutely lame horse has an abscess. Pedal-bone fracture, deep puncture, infection of the coffin joint or navicular bursa, severe bruising and soft-tissue injury inside the foot can produce the same dramatic pain. Deep tracks are more dangerous because bacteria can reach synovial structures or bone, while excessive removal of protective sole can create additional injury without relieving the original pressure.
Simple superficial abscesses carry an excellent prognosis once pressure is released and clean horn regrows. Recurrent drainage from one site instead suggests a persistent defect such as a keratoma, dead bone, chronic laminar separation, white-line disease or deeper infection. Continued severe pain after drainage is inconsistent with an uncomplicated pocket and raises concern for remaining pressure or a more serious lesion.
Pus follows the path of least resistance and may travel upward through the hoof wall before opening at the coronary band. This can temporarily relieve pressure while leaving a long tract that must grow out with the horn. Repeated wetting and drying, thin soles, old nail holes and distorted white lines increase entry opportunities, especially when hoof quality or laminar attachment is already compromised.
Examination compares digital pulses, hoof temperature, coronary-band swelling and heel-bulb pain. The sole is cleaned, and hoof testers are applied systematically. A wire brush and conservative hoof-knife exploration can reveal a dark tract or soft focus corresponding to pain; normal horn is preserved.
If no tract is found, a poultice for 24–48 hours can soften the sole before re-examination. Persistent severe lameness warrants radiographs to detect gas, a foreign body, pedal-bone fracture, keratoma or bone infection. Nerve blocks localise pain to the foot but do not distinguish a superficial abscess from every deeper lesion. CT or MRI is reserved for unresolved or recurrent cases.
Any puncture is mapped radiographically with the object or a sterile marker before the tract is enlarged. Fever, swelling above the hoof, synovial fluid leakage or a wound directed toward the frog’s central region raises concern for deep infection and requires emergency referral. A presumed abscess that remains markedly lame after adequate drainage should be reassessed within hours, not left through repeated soaking for days.
Treatment creates the smallest effective dependent drainage opening while preserving weight-bearing sole. Once a tract is accurately located, limited paring releases pus. The foot is then poulticed and bandaged for several days to maintain drainage and keep debris out. NSAIDs provide pain relief and encourage comfortable weight bearing, which assists evacuation.
Routine systemic antibiotics are unnecessary for a simple superficial abscess after drainage and may slow localisation. They are indicated when infection extends into deeper dermis, bone or a synovial structure, guided by culture and surgical management. Tetanus protection is updated when needed. If the abscess drains at the coronary band, a sole route may still be sought to allow dependent drainage without extensive wall removal.
Marked improvement should occur within about 12–24 hours. Failure prompts repeat examination and imaging. The drainage site is protected until dry and firm, then the shoe is replaced or a pad used as appropriate. Recurrent abscesses require correction of thin soles, white-line separation, laminitis or hoof imbalance. Hot nails and deep punctures receive a more aggressive plan because joint, bursa, tendon and pedal bone infection can be career- or life-threatening.
