Stringhalt is a neuromuscular gait disorder in which one or both hindlimbs flex upward excessively during the forward swing phase. The movement ranges from a brief exaggerated snap to the hoof striking the abdomen before slamming down. Signs are most obvious at the walk, when turning sharply or backing, may lessen at the trot and are usually absent at canter.
Classic stringhalt occurs sporadically in individual horses and often affects one limb; its cause is frequently unknown. Australian stringhalt tends to affect both hindlimbs and can occur in groups grazing drought-stressed pastures containing flatweed or related plants. The toxic form is associated with peripheral nerve injury and may include laryngeal dysfunction. Severity can fluctuate with temperature and activity.
Stringhalt is a mechanical-looking gait abnormality rather than ordinary painful lameness. Foot pain or lower-limb irritation can produce “false stringhalt”, and upward fixation of the patella, shivers and fibrotic myopathy have different movement patterns. In shivers, spasm is most characteristic during backing or foot lifting and is accompanied by abduction and tail elevation; stringhalt is usually visible in forward walking.
Mild cases may not prevent useful work, while severe bilateral flexion makes walking, handling and farriery unsafe and can cause secondary concussion injury. Many Australian stringhalt horses improve after removal from affected pasture, but nerve recovery can take months to years. Classic cases are less predictable. The disease is not contagious, although several horses can be exposed to the same toxic pasture.
The exaggerated step reflects abnormal activation within peripheral motor pathways rather than a deliberate response. Muscles can contract in the correct sequence but with excessive intensity and poor timing, producing the characteristic snap. Excitement, cold, turning and the first steps after standing can amplify the movement. With Australian stringhalt, wider nerve injury may also affect the larynx and muscle condition during a prolonged recovery.
The horse is observed walking in a straight line, turning tightly, backing and moving after rest. Severity, laterality, pasture history, season and whether herd mates are affected are documented. Video is valuable because mild stringhalt can be intermittent. Examination includes the feet, pastern skin and limbs to exclude painful false stringhalt.
Neurological and lameness assessment distinguishes peripheral neuropathy from patellar locking, fibrotic myopathy and shivers. Electromyography can demonstrate denervation in selected cases, but the characteristic gait and exposure pattern usually drive diagnosis. Laryngeal endoscopy is indicated when Australian stringhalt is accompanied by respiratory noise or swallowing concerns.
Pasture survey looks for flatweed and other implicated plants, particularly after drought or overgrazing. Plant identification should be confirmed rather than based on a common name alone. Blood tests are generally nonspecific and there is no routine toxin assay that rules disease in or out. A cluster of bilateral cases on one pasture strongly supports an environmental cause and warrants immediate removal of all exposed horses.
For suspected Australian stringhalt, horses are removed from the implicated pasture and supplied safe forage without waiting for complete plant eradication. Pasture rehabilitation addresses overgrazing and weed dominance. Recovery is spontaneous in many horses but slow because peripheral nerves must regenerate; severe cases need safe housing, assistance and monitoring for laryngeal dysfunction.
Medication such as phenytoin or muscle relaxants has been tried with variable temporary benefit. Correcting a documented nutritional deficiency supports recovery but unproven supplements do not remove the toxic exposure. Exercise is limited to what the horse can perform without falling or repeated hoof trauma, then increased as the gait improves.
Chronic classic stringhalt may be treated surgically by removing a portion of the lateral digital extensor muscle and tendon. Improvement is not guaranteed and may take weeks, so case selection and rehabilitation matter. False stringhalt resolves only when the painful foot or pastern lesion is treated. Hoof care is staged with calm restraint when hyperflexion creates risk. Prognosis is favourable for many toxic cases after pasture removal, but persistent severe gait or associated laryngeal paralysis can make humane euthanasia necessary.
