Squamous cell carcinoma, or SCC, is a malignant tumour of surface epithelial cells. In horses it commonly affects the eyelids, third eyelid, conjunctiva and corneal edge, as well as penis, prepuce, vulva and other lightly pigmented or chronically irritated skin. Ultraviolet exposure, age, low local pigmentation, papillomavirus infection at some genital sites and inherited susceptibility in certain breeds influence risk.
Lesions may begin as a small rough plaque, non-healing ulcer or pink proliferative mass that bleeds easily. Ocular SCC causes tearing, discharge, squinting and a mass near the limbus or third eyelid. Penile disease can produce smegma, foul discharge, bleeding, swelling, difficulty urinating or a mass hidden within the sheath. Horses can tolerate slow growth until invasion is extensive.
SCC is locally destructive and can spread through lymphatic vessels to regional nodes and later distant sites. It is not equivalent to a sarcoid or proud flesh, and repeated superficial removal delays effective margin control. Early lesions are much easier to treat while preserving the eye or penis.
Prognosis depends on site, size, depth, lymph-node involvement and first-treatment quality. Small ocular or genital lesions treated with complete excision plus an adjunct often have good control. Recurrent, orbital, urethral or nodal disease is more guarded. Prevention cannot eliminate all risk, but UV protection and routine inspection of pale eyelids and genital tissue support earlier detection.
Equine SCC favours poorly pigmented, lightly haired or chronically irritated tissue, particularly eyelids, third eyelid, penis, prepuce and vulva. The tumour invades locally and may metastasise to regional lymph nodes. Ocular discharge or a non-healing genital sore can appear minor while deeper invasion is already limiting the possibility of complete excision.
Earlier, smaller lesions offer more tissue-sparing treatment choices and better function.
Any persistent proliferative, ulcerated or bleeding lesion receives biopsy for histopathology. Ocular examination evaluates eyelids, conjunctiva, third eyelid, cornea and orbit; fluorescein staining and fundoscopy assess concurrent eye disease. Sedated extension of the penis and complete sheath examination are essential when discharge or swelling is unexplained.
The veterinarian palpates regional lymph nodes and samples enlarged nodes by aspiration or biopsy. Ultrasound defines penile depth, while orbital or extensive head disease may require CT or MRI. Thoracic or abdominal imaging is considered when metastasis is suspected. Papillomavirus testing has selected value but does not replace histology.
Owners should inspect pale ocular margins and the sheath or vulva routinely, documenting any plaque that persists beyond normal wound healing. A lesion treated empirically that returns deserves biopsy before another destructive procedure. Straining to urinate, globe displacement, severe ocular pain or rapid nodal enlargement indicates advanced disease and needs urgent referral.
Regional lymph nodes are palpated and sampled when enlargement could represent metastatic spread.
Treatment aims for complete local control while preserving function. Small ocular lesions may undergo superficial keratectomy, third-eyelid excision or eyelid surgery, commonly combined with cryotherapy, local chemotherapy, hyperthermia or beta radiation. Adjunctive treatment matters because excision alone leaves a higher recurrence risk. Advanced orbital invasion can require removal of the eye and orbital contents.
Penile and preputial lesions are treated with local excision, laser, cryotherapy or topical or intralesional chemotherapy when small. Partial phallectomy or more extensive reconstructive surgery is needed for invasive disease, with attention to urethral flow. Confirmed lymph-node metastasis may require node removal and additional therapy.
Pain control, fly protection and clean wound management support healing. Follow-up is frequent during the first year and continues lifelong because recurrence or new lesions can develop. UV-blocking masks and shade reduce exposure for at-risk ocular tissue but do not treat an existing tumour. Prognosis is best at first treatment of a small lesion; repeated incomplete procedures create scarring and allow deeper invasion, sometimes leaving euthanasia as the only welfare-protective option.
Margins and lymph-node status determine whether local therapy has a realistic chance of durable control.
