A “splint” is inflammation and new bone formation involving a small metacarpal or metatarsal splint bone and its interosseous attachment to the cannon bone. It most commonly appears along the upper inside forelimb in a young horse entering work. Repetitive loading, direct interference, hoof imbalance and concussion can strain the attachment before the bones have fully stabilised together.
An active splint produces local heat, pain and a firm swelling, with lameness that may be obvious only during work. As inflammation settles, a hard bony prominence remains. Many mature, inactive splints are cosmetic and painless. Position matters: a large or inward-growing mass can impinge on the suspensory ligament, while a fracture of the splint bone—often after a kick—has different treatment and prognosis.
The term should not be applied to every lump beside the cannon. Suspensory desmitis, splint-bone fracture, infection, exostosis from trauma and sequestrum can look similar. In hindlimbs, wounds and fractures are particularly common after kicks. A small external bump can also overlie more substantial ligament injury.
Most uncomplicated active splints settle with time and controlled return to work, though the bony enlargement rarely disappears completely. Future athletic prognosis is good when the suspensory ligament is unaffected. Recurring pain, rapid enlargement, a draining wound or a mass crowding the ligament warrants imaging rather than repeated blistering or cosmetic treatment.
A splint is inflammation and new bone along a splint bone, usually the inside of a young horse's fore cannon. Early lesions are painful before the firm swelling matures. Most become clinically quiet, but a large or strategically placed exostosis can impinge the suspensory ligament or a joint and cause persistent lameness.
A fracture of the splint bone is a separate injury and needs different assessment.
The veterinarian palpates the entire splint bone and suspensory region for heat, focal pain, swelling and wounds, comparing the opposite limb. Gait is evaluated on suitable surfaces, and diagnostic nerve blocks may be needed if lameness localisation is uncertain. Hoof balance and evidence of limb interference are recorded.
Radiographs distinguish periosteal new bone from a splint-bone fracture, infection or a lesion involving the cannon. Oblique views are often required. Ultrasound assesses the adjacent suspensory ligament and soft tissues, particularly when the mass projects inward or lameness exceeds the visible swelling. A wound over a splint bone is examined for sequestrum and contamination.
Serial photographs or measurements show whether an active swelling is enlarging. A mature cold splint found during prepurchase examination should be interpreted by location, pain and imaging rather than rejected automatically. Persistent focal pain after a period of rest, recurrent swelling on resuming work or any discharge is a reason for repeat radiography and ultrasound.
Ultrasound assesses suspensory impingement when the swelling lies close to the ligament.
Acute uncomplicated inflammation is managed by reducing exercise, cold therapy and a short veterinarian-directed course of anti-inflammatory medication. A controlled return begins after heat, pain and lameness resolve. Hoof balance, shoe fit and interference are corrected so the original stress is not repeated. Prolonged complete rest is rarely necessary for a minor periosteal reaction but training through active pain encourages a larger lesion.
Topical counterirritants and old-fashioned firing do not remove established bone and can damage skin. Corticosteroid around an active lesion is used selectively and must avoid the suspensory ligament. The hard bump usually remains even when clinically resolved; treatment should pursue soundness, not cosmetic disappearance.
A fractured distal splint-bone segment may be surgically removed when painful, displaced or associated with a wound, while proximal fractures can require fixation because too much removal destabilises the carpus or hock. Exostosis impinging on the suspensory ligament can be resected with careful rehabilitation. Antimicrobials and debridement are needed for infected bone. Prognosis is excellent for a simple splint and more dependent on associated suspensory injury than on the size of the external lump.
