Small-intestinal obstruction prevents fluid, gas and ingesta from moving normally through the duodenum, jejunum or ileum. A simple obstruction blocks the lumen, while a strangulating obstruction also compresses the intestinal blood supply. Causes include pedunculated lipomas, entrapment through natural or acquired openings, volvulus, intussusception, adhesions and firm ileal impaction. Strangulation is an emergency because deprived intestine becomes swollen and nonviable within hours.
Pain ranges from intermittent to severe and unrelenting. Horses may paw, roll, sweat, stop eating and pass little manure. Fluid secreted into obstructed small intestine backs up into the stomach, producing nasogastric reflux. Because horses cannot vomit, gastric decompression can be lifesaving. As fluid leaves the circulation, dehydration, rapid heart rate, abnormal gums and shock develop. A horse may become deceptively quiet when intestinal tissue is severely compromised, so reduced struggling does not always indicate improvement.
Older horses have greater risk of strangulation by a fatty pedunculated lipoma, whereas young horses are more likely to develop some congenital entrapments or intussusception. Ileal impaction can be associated with coarse forage, parasites or altered motility and may occasionally respond to medical treatment if recognised before substantial reflux and distension develop.
Mechanical obstruction and strangulation cannot be prevented reliably through routine husbandry. The decisive issue is early recognition and referral. Prolonged distension damages the intestinal wall, while ischaemic tissue releases inflammatory products that promote endotoxaemia, postoperative ileus and adhesions. Even a surgically corrected obstruction can therefore require prolonged hospital care, and later adhesion formation may cause recurrent colic. Prognosis depends on the lesion, duration, length of damaged bowel and whether circulation can be restored before irreversible injury occurs. Owners should understand that early exploratory surgery may be the treatment that prevents a definitive diagnosis from arriving too late.
There is no screening test for an impending small-intestinal strangulation. Any colic with persistent pain, repeated need for analgesia or substantial gastric reflux warrants urgent assessment. Owners should report the speed of onset, pain pattern, manure production, previous abdominal surgery, age and any earlier episodes. Repeatedly giving pain medication without reassessment can delay the surgery needed to save viable intestine.
Veterinary examination includes cardiovascular assessment, rectal palpation and nasogastric intubation. Distended small-intestinal loops may be felt rectally or seen with abdominal ultrasound; wall thickness and motility add information about severity. Blood lactate, hydration and electrolyte changes reflect systemic effects, while abdominal-fluid protein, cell count, colour and lactate can support concern for compromised bowel. Normal abdominal fluid early in the episode does not exclude strangulation.
Referral is indicated when pain continues despite appropriate medication, pain returns rapidly, reflux accumulates, small intestine remains distended or circulatory findings worsen. Serial examinations can clarify an uncertain case, but observation must have a defined time limit. Some lesions are confirmed only during exploratory surgery, and waiting for complete diagnostic certainty can reduce survival.
Initial treatment stabilises circulation and prevents gastric rupture while preparations for referral or surgery proceed. A nasogastric tube is used repeatedly when reflux accumulates. Intravenous fluids and electrolytes replace losses into the intestine, and analgesia is selected to maintain welfare without obscuring deterioration. Enteral fluids should not be administered when reflux or small-intestinal obstruction prevents absorption.
Most mechanical and strangulating lesions require emergency abdominal surgery. The surgeon identifies and releases the obstruction, evaluates intestinal colour, thickness, motility and blood flow, and removes nonviable bowel where feasible. An ileal impaction detected before severe compromise may sometimes receive intensive medical treatment, but worsening pain, reflux or distension changes that decision. Antimicrobials, anti-inflammatory care and measures addressing endotoxaemia are used according to contamination and tissue injury.
After surgery, monitoring focuses on pain, reflux, intestinal motility, incision healing, thrombosis, adhesions and laminitis. Feed is reintroduced cautiously once function returns. Controlled walking and later exercise follow an abdominal-surgery rehabilitation plan. Postoperative ileus can require continued decompression, fluids and prokinetic treatment. Prognosis is best when viable intestine is restored promptly; extensive resection, severe shock, peritonitis or prolonged illness makes both short- and long-term survival more guarded.
