Sand enteropathy develops when ingested sand or fine soil accumulates in the large intestine. Horses may take in sand while grazing sparse pasture, eating hay or concentrate from bare ground, pulling short plants from sandy soil or deliberately eating dirt. Drought and overgrazing increase exposure. Small quantities can pass normally, but a heavy burden settles in the ventral colon, where its weight and abrasive action disturb motility and inflame the lining.
Affected horses may develop intermittent colic, chronic or acute diarrhoea, weight loss, reduced performance and poor condition. A large accumulation can form an impaction and completely obstruct the colon. The weight of sand may stretch the supporting tissues, while mucosal abrasion contributes to inflammation and protein loss. Severe obstruction resembles other surgical colics and cannot be diagnosed from environmental exposure alone.
Risk is property-specific. Several horses sharing a sandy, depleted paddock may be exposed, but individual burdens differ according to feeding position, grazing behaviour and gut transit. Finding a little sand in manure confirms ingestion, not necessarily clinically important accumulation. Conversely, a horse with a heavy retained burden may pass an inconsistent amount in feces.
Early disease often responds well when the sand source is removed and the burden is cleared. Prolonged exposure can lead to recurrent enterocolitis, persistent diarrhoea or obstruction requiring surgery. Sand accumulation should be distinguished from the incidental grit that many horses pass after brief exposure; clinical importance depends on retained quantity and intestinal effects. Prevention relies primarily on reducing ingestion: provide adequate forage, use feeders or mats that keep food away from soil, and address overstocking or barren paddocks. Psyllium may assist removal in selected programmes, but it cannot compensate for continued daily ingestion.
Assessment starts with a detailed environmental and feeding history. Owners should note whether forage is fed directly on sandy ground, whether pasture is sparse, and whether other horses have diarrhoea or colic. A fecal sediment test can demonstrate sand passage but has limited ability to quantify retained burden. Repeated negative samples do not completely exclude accumulation.
Veterinarians may hear characteristic gritty or “sand” sounds over the lower abdomen, although this finding is not present in every case. Abdominal radiography is the most useful way to visualise and estimate a large-colon sand burden where equipment can penetrate the horse’s abdomen. Ultrasound, rectal examination, blood tests and abdominal-fluid analysis help assess impaction, inflammation and competing diagnoses. Trends in weight and manure consistency are useful during follow-up.
Urgent assessment is needed for persistent pain, abdominal distension, absent manure, fever, dehydration or severe diarrhoea. Horses on the same property should be reviewed for shared exposure, but treatment decisions should be based on each animal’s findings. Follow-up radiographs or clinical reassessment can determine whether the burden is actually declining rather than assuming that visible sand in manure proves clearance.
The first treatment step is preventing further ingestion by moving the horse or changing how feed is offered. Psyllium husk is commonly administered in a veterinarian-directed course to help suspend and carry sand through the colon. It may be given in feed or through a nasogastric tube, and palatability and adequate hydration matter. Large burdens frequently require treatment over several weeks rather than a single monthly dose.
Horses with colic, diarrhoea or impaction may also need analgesia, enteral or intravenous fluids and correction of electrolyte or protein losses. Treatment intensity depends on pain, hydration, intestinal motility and the amount and location of sand. Complete obstruction, worsening distension or failure of medical clearance can require surgery, with sand removed through an enterotomy. Heavy sand increases the difficulty of handling the colon and can be associated with bowel damage.
Manure, appetite, pain and hydration should be followed during treatment, with imaging repeated when the original burden was substantial. Diarrhoea may improve before all sand has left the bowel. Long-term prevention is environmental: feed from raised tubs, large rubber mats or other clean surfaces, maintain adequate forage coverage and prevent horses from searching bare soil for feed. Preventive psyllium can be discussed in high-risk areas, but evidence and dosing plans should be reviewed locally rather than used as permission to keep feeding on sand.
