Ringworm, or dermatophytosis, is a contagious superficial infection of hair and keratinised skin by fungi such as Trichophyton equinum. Despite the name, no worm is involved. Spores pass through direct horse contact and contaminated brushes, rugs, tack, clippers, rails and vehicles, remaining viable in the environment for prolonged periods.
Lesions often begin as raised hair tufts, then become areas of broken hair, scaling and crusting. Circular shape is common but not required, and multiple patches can merge. Saddle and girth regions are frequently affected because friction inoculates spores. Itching and inflammation vary with fungal species and the horse’s immunity. Young, stressed or crowded horses are most susceptible.
Many healthy horses eventually self-cure, but untreated animals contaminate equipment and expose people and other horses. Dermatophytosis is zoonotic, causing itchy ring lesions in humans. Rain scald, bacterial folliculitis, mites and immune-mediated scaling can look similar; treating every round bald patch as ringworm can prolong the wrong therapy.
Diagnosis and environmental control matter more than applying a spot cream to the most visible circle. Microscopic disease extends beyond lesion edges, and new patches can appear while contaminated tack is reused. Prognosis is excellent, although coat regrowth lags behind fungal cure. Competition or facility return should follow local biosecurity rules and objective clinical or mycological recovery, not one day without new lesions.
Dermatophyte fungi invade keratinised hair and superficial skin, producing expanding circular or irregular areas of scaling and hair loss. Lesions may be only mildly itchy, and early disease can resemble rubs or bacterial folliculitis. Infective spores persist on timber, rugs and brushes for long periods and can also infect people.
Young or stressed horses tend to develop more conspicuous lesions. Lesions commonly spread before spontaneous immunity develops.
Early active lesion margins are sampled before topical treatment. Hairs are plucked and scales and crusts collected for direct microscopy, fungal culture or validated PCR. The site should not be wiped with alcohol because this can reduce culture yield. A negative test after poor sampling does not exclude infection, and repeat or biopsy may be needed for atypical disease.
The entire horse is inspected under good light, including girth, saddle and face, and in-contact horses are checked. Brushes, rugs, tack and stable movements are traced to identify spread. Human skin lesions are referred to a medical professional rather than diagnosed by the veterinarian.
Differentials are investigated with cytology, bacterial culture, skin scrapings or biopsy as indicated. During an outbreak, suspected horses are isolated and equipment dedicated before confirmation. Follow-up culture or PCR can document mycological cure in high-risk facilities; hair regrowth alone is slow and an unreliable release measure.
Newly plucked hairs from the active edge provide a better fungal sample than old bare centres.
Topical whole-body treatment is preferred because spores extend beyond visible lesions. Effective protocols include leave-on lime-sulfur or enilconazole rinses, or miconazole with chlorhexidine shampoo, applied at veterinarian-directed dilution and frequency. Loose crust and hair are removed gently and disposed of safely. Spot treatment alone often misses early foci.
Systemic antifungals are rarely necessary and can be costly or toxic. Therapy continues until clinical and, where required, mycological cure. Bleach should not be applied to the horse’s skin. Personnel use gloves, protective clothing and hand hygiene.
Environmental decontamination is essential: organic hair and dirt are removed first, hard surfaces and equipment washed with detergent, then an antifungal disinfectant is applied for the labelled contact time. Fabric rugs and leads are laundered thoroughly; porous items that cannot be cleaned may need disposal. Each horse has dedicated tack. Isolation ends according to facility and veterinary criteria. Recurrence usually reflects missed carriers or contaminated equipment rather than drug resistance, so outbreak mapping and cleaning are reviewed before escalating medication.
Handlers wear gloves and treat exposed human lesions through their own healthcare professional. Topical treatment covers beyond visible lesion margins because infected hairs extend into the surrounding coat.
