Ringbone is osteoarthritis and periarticular new bone involving the pastern or coffin region. High ringbone affects the proximal interphalangeal, or pastern, joint; low ringbone affects the distal interphalangeal, or coffin, joint. The popular name describes the bony response around the joint rather than one specific cause.
Repeated concussion, poor limb or hoof alignment, joint instability, fracture, cartilage injury and developmental disease can initiate degeneration. Horses develop progressive lameness, reduced pastern motion, pain on flexion and sometimes a firm bony enlargement above the coronary band in high ringbone. Low ringbone is hidden inside the hoof and may have no visible swelling. Sidebone—ossification of the collateral cartilages—is anatomically different and not automatically painful.
High-motion coffin-joint disease is difficult because normal foot function depends on continued movement at that level. The pastern joint moves less and can sometimes become comfortable after complete fusion. New bone develops as the joint attempts to stabilise abnormal motion, but the amount visible around the joint does not necessarily reflect the activity of inflammation or the horse’s degree of pain.
Ringbone is chronic and generally progressive, but rate and functional effect vary. Early correction of imbalance and workload may slow deterioration. Prognosis is guarded for high-level work with coffin-joint disease, while surgical pastern arthrodesis can provide a good chance of comfortable use when alignment and surrounding structures are suitable. Established deformity and cartilage loss cannot be reversed by a supplement or a single joint injection.
Reduced pastern movement changes breakover and loading through the whole limb. A horse may shorten the stride, land unevenly or overload the opposite forelimb before enlargement becomes obvious. Disease extending into an articular surface is more likely to remain painful than new bone confined outside the joint, while hoof imbalance can perpetuate stress at either level.
A lameness examination evaluates pastern conformation, hoof balance, range of motion, firm swelling and response to flexion. Gait is assessed on suitable straight lines and circles. Sequential nerve blocks and, when necessary, intra-articular analgesia distinguish pastern or coffin-joint pain from navicular, collateral-ligament and other foot disease.
Radiographs show osteophytes, joint-space loss, subchondral sclerosis, cystic change and alignment. Oblique views define asymmetric new bone; CT or MRI can identify occult fracture, ligament injury or early coffin-joint disease when standard images are inconclusive. Synovial fluid is sampled when sudden effusion, heat, wound or recent injection raises concern for infection.
Both limbs are imaged when conformation or bilateral signs warrant comparison. Hoof photographs and marked lateral radiographs guide farriery. Screening should not label every periarticular spur as clinically significant without pain localisation. Rapidly worsening lameness or a wound near the joint requires urgent investigation rather than assuming routine chronic ringbone.
Targeted nerve or joint blocks distinguish pastern-joint pain from foot and soft-tissue disease.
Early management reduces joint inflammation and abnormal leverage. Work is modified, body weight controlled and hoof balance corrected. A shortened or rolled breakover and support matched to the affected joint can lessen torque. NSAIDs and intra-articular corticosteroid, hyaluronic acid or selected biologic products provide symptom control when infection has been excluded and the painful joint confirmed.
Low ringbone treatment resembles other coffin-joint osteoarthritis care and may include shockwave or other adjuncts, but fusion is not a practical routine goal. Persistent high ringbone can be treated by surgical pastern arthrodesis using screws and plates, with cartilage removal and bone grafting as indicated. Correct alignment, rigid fixation and a protected recovery allow the low-motion joint to unite and eliminate painful movement.
Postoperative monitoring looks for implant infection, delayed union and supporting-limb overload. Rehabilitation is prolonged and guided by radiographic fusion. Chemical or facilitated ankylosis without stable alignment is less predictable. The horse may return to useful work after successful pastern fusion, though the discipline may change. Low ringbone and advanced coffin-joint cartilage loss carry a more guarded outlook and often require lifelong management focused on comfort.
