Rhodococcus equi pneumonia is a chronic, abscess-forming bacterial lung infection seen mainly in foals between about one and five months of age. R. equi is widespread in soil and manure on many horse properties; disease reflects inhalation of virulent organisms by a susceptible young foal rather than simple exposure alone. The bacteria survive inside macrophages, allowing infection to build slowly and form areas of lung consolidation and pyogranulomatous abscessation.
Early signs can be deceptively mild because foals compensate for substantial lung damage. A slightly increased resting respiratory rate, reduced play, intermittent fever or poor weight gain may precede cough, nostril flare and obvious breathing effort. Nasal discharge is inconsistent. Advanced disease can cause respiratory distress, hypoxaemia and death. Some foals also develop abdominal abscesses, diarrhoea, infected joints, bone infection or immune-mediated joint swelling.
Large breeding farms, dusty paddocks, high foal density and a previous history of clinical disease can increase risk, but detection of R. equi in the environment does not predict which individual will become ill. Likewise, finding the organism in an airway sample without compatible pneumonia does not automatically justify prolonged antibiotics.
The condition differs from acute viral respiratory outbreaks in adult horses. It usually progresses within individual young foals and requires confirmation of invasive disease. Prognosis is favourable for many foals treated before severe respiratory compromise, while extensive lung abscessation, extrapulmonary infection and delayed recognition worsen the outlook. Recovered uncomplicated foals can often achieve normal athletic performance, but the treatment period is long and must be guided by objective resolution rather than disappearance of fever alone.
Foals usually inhale or swallow R. equi from dusty soil and manure during the first weeks of life, but pneumonia becomes apparent later as abscesses enlarge. They can compensate surprisingly well, so extensive lung disease may exist before coughing or respiratory effort is noticed. Extrapulmonary abscesses, diarrhoea, joint disease and immune-mediated polysynovitis alter prognosis and treatment.
On properties with recurring R. equi disease, a veterinarian may design age-specific surveillance using regular physical examination and thoracic ultrasonography. Resting respiratory rate and effort, temperature, growth and activity are useful, but blood fibrinogen, white-cell count and serum amyloid A are not sufficiently specific to stand alone. Ultrasound detects peripheral consolidation yet can miss lesions deep beneath aerated lung.
A foal with suspicious findings should receive a complete examination and thoracic imaging. Radiographs can identify deeper, perihilar or nodular disease and mediastinal lymph-node enlargement. Definitive diagnosis requires evidence of pneumonia plus a tracheobronchial aspirate submitted for cytology, bacterial culture and testing for the virulence-associated VapA gene. Culture also identifies mixed infection and permits antimicrobial susceptibility testing.
Serology is not recommended for diagnosing active disease. Screening programmes should avoid automatically treating every small ultrasound lesion because many regress and unnecessary macrolide-rifampin exposure selects resistant organisms. Serial lesion size, breathing, fever and clinical condition determine whether close observation or treatment is appropriate. Abdominal pain, diarrhoea, joint swelling or lameness should prompt investigation for extrapulmonary infection.
Confirmed clinically important R. equi pneumonia usually requires prolonged combination antimicrobial therapy. A macrolide such as clarithromycin or azithromycin is commonly paired with rifampin because both penetrate cells and abscess material. Drug choice, dose and duration belong under veterinary control and should be refined if culture demonstrates resistance or a mixed bacterial infection. Severe cases may require treatment for many weeks.
Support includes a clean, cool, well-ventilated low-dust environment, good nutrition, hydration and anti-inflammatory medication when fever or discomfort warrants it. Foals with hypoxaemia need oxygen, and marked respiratory effort may require hospital care. Heat stress is a recognised complication of macrolide treatment, so temperature, breathing and weather exposure must be monitored. Diarrhoea can occur in the foal, and macrolide exposure from the treated foal can cause life-threatening colitis in the mare; safe handling and prompt investigation of maternal diarrhoea are essential.
Progress is followed with clinical examination and repeat thoracic ultrasound or radiographs. Treatment is not stopped solely because cough or fever resolves; consolidation and abscessation should show appropriate resolution. Unconfirmed preventive antibiotic dosing of healthy foals is discouraged because of resistance and adverse effects. Farms with recurrent disease should reduce dust and crowding and review surveillance with their veterinarian rather than relying on environmental cultures or indiscriminate medication.
