Pituitary pars intermedia dysfunction, or PPID, is a progressive endocrine disorder of older horses and ponies. Degeneration of dopamine-producing nerves removes normal inhibition from the pituitary pars intermedia, allowing it to enlarge and produce excessive peptide hormones including ACTH. The older name “equine Cushing’s disease” is imprecise because the hormone pattern differs from canine and human Cushing syndromes.
The most characteristic advanced sign is a long, sometimes curly coat that fails to shed normally. Earlier changes can include delayed shedding, loss of topline muscle, a pendulous abdomen, lethargy, abnormal sweating, increased drinking and urination, fat above the eyes and recurrent infections such as hoof abscesses, sinus or dental disease. Tendon and ligament problems may be more frequent. Some horses lose weight rather than becoming obese.
Laminitis risk is greatest when PPID is accompanied by insulin dysregulation; PPID alone does not explain every laminitis case. The condition can also coexist with dental disease, chronic pain and ordinary ageing, so a long coat or muscle loss should not end the diagnostic investigation. Conversely, early PPID may be present before obvious hypertrichosis.
PPID cannot be cured, but medical control and attentive husbandry can materially improve comfort and longevity. Progression and seasonal hormone changes mean dose needs and test results evolve over time. Prognosis depends less on the pituitary enlargement itself than on laminitis, infection, weight loss and the owner’s ability to monitor and manage an ageing horse consistently.
An age-related loss of dopaminergic control allows the pars intermedia to overproduce several hormones, including ACTH. Delayed coat shedding is classic but often late; earlier clues include loss of topline, abnormal fat distribution, recurrent infections, increased drinking and laminitis. Seasonal ACTH variation and assay-specific reference intervals are essential when interpreting borderline results.
Testing is recommended for compatible signs and can be incorporated into senior-horse assessment. Resting plasma ACTH is widely used, with samples handled correctly and interpreted against season-specific laboratory ranges because healthy horses have a natural seasonal rise. A clearly high result in a compatible horse supports PPID, while an equivocal result may need repetition.
The thyrotropin-releasing hormone stimulation test can identify earlier disease by measuring the ACTH response after TRH, using season- and protocol-specific cut-offs. Acute pain, severe illness and sample handling can distort endocrine values. Older dexamethasone suppression protocols are used less often and require careful selection.
Every PPID horse should be assessed for insulin dysregulation using resting and, when appropriate, dynamic testing because this determines laminitis risk. Examination also includes teeth, body and muscle condition, hooves, parasite control and evidence of infection. After treatment begins, clinical response and ACTH are reassessed at an appropriate interval and then periodically. A normal coat after clipping is not evidence of hormonal control, and one normal early test does not permanently exclude progressive disease.
Pergolide, a dopamine agonist, is the treatment of choice and is given daily at an individually titrated dose. Improvement in alertness and excessive drinking may appear within weeks, while coat and body composition take longer. Temporary reduced appetite or dullness can occur when treatment starts; the veterinarian may adjust or reintroduce the dose rather than allowing unmonitored discontinuation.
Hormone results, clinical signs and season guide dose review. Treatment is usually lifelong, and abruptly missing repeated doses allows endocrine activity to return. Pergolide does not by itself correct insulin dysregulation, laminitis, infection or dental pain. Those problems receive separate management, including low-sugar forage where indicated, therapeutic hoof care and prompt investigation of infections.
Senior-horse husbandry is essential: regular dental care, parasite assessment, vaccination, hoof care, body-weight tracking, adequate quality protein and clipping of an insulating coat in warm weather. Feed is adapted if chewing is poor or weight is falling. Exercise matches soundness and muscle condition. Horses with concurrent insulin dysregulation require stricter pasture control and follow-up insulin testing. With consistent medication and care, many PPID horses maintain good quality of life for years.
