Periodontal disease affects the gum, periodontal ligament, cementum and bone supporting a horse’s teeth. It often begins when forage becomes packed into abnormal spaces between cheek teeth. Fermentation and bacterial contamination inflame the gum, deepen the pocket and progressively destroy attachment around the tooth. Diastemata, misaligned teeth, age-related changes and abnormal wear make feed trapping more likely.
Affected horses may have bad breath, quidding, slow chewing, weight loss, drooling or resistance to the bit. Pain can be substantial even when appetite remains. Feed may be visible between teeth, and inflamed gums can bleed when probed. Advanced disease loosens the tooth, damages surrounding bone and can spread to a root or paranasal sinus. Periodontitis is therefore different from simple superficial gingivitis and cannot be corrected by smoothing sharp points alone.
The condition is common in older horses but can affect younger animals with developmental spaces or displaced teeth. Once supporting tissue and bone are lost, they do not return completely. Early treatment aims to stop feed packing and preserve a comfortable functional tooth; late treatment may require extraction. Dental pain can worsen body condition and increase choke risk through poor mastication.
Periodontal disease is localised, so one side or one interdental space may be severe while the rest of the mouth appears reasonable. This makes casual visual inspection unreliable. Prognosis depends on pocket depth, tooth stability, root involvement and whether the mechanical reason for packing can be corrected.
Feed packed into an abnormal gap ferments against the gum, deepening a painful periodontal pocket and progressively loosening the tooth. A foul odour or unilateral nasal discharge may appear when an upper cheek-tooth infection extends into a sinus. Disease is often focal, so an apparently acceptable grinding surface does not exclude a deep pocket between adjacent teeth.
Screening requires a sedated full-mouth examination with a speculum, light and systematic cleaning of the cheek-tooth arcades. Owners should arrange earlier assessment for persistent odour, quidding, long fibre in manure, unexplained weight loss, facial swelling or unilateral nasal discharge. Routine incisor inspection and feeling the outer cheek cannot exclude disease deep between teeth.
After packed feed is removed, the veterinarian probes pocket depth, bleeding, attachment loss and tooth mobility. Dental radiographs show bone loss, root infection and changes that affect whether the tooth can be saved. Oral endoscopy improves visualisation of narrow diastemata and the back of the mouth. CT may be used for complex root or sinus disease.
Repeated charting and photographs document whether a treated pocket is healing or continuing to trap feed. Culture is not routinely needed for every pocket because the mechanical lesion must still be corrected. Screening intervals are shortened for horses with established diastemata, missing teeth or previous extractions, since recurrence and new overgrowths are common.
Treatment begins by removing impacted feed and thoroughly cleaning the periodontal pocket. The veterinarian then addresses the anatomy that allows material to re-enter. Selected diastemata may be widened to create an open channel that clears during chewing, while focal overgrowths are carefully reduced to restore movement. Other cases benefit from packing material or staged periodontal procedures. Repeated cleaning alone is unlikely to succeed if the trapping mechanism remains.
Pain relief and local treatment are used according to pocket depth and infection. Systemic antimicrobials are reserved for spreading infection, root disease or another clear indication; they cannot rebuild lost attachment or remove impacted forage. A loose, fractured or severely unsupported tooth may need extraction. The socket and any associated sinus disease then require follow-up, and the opposing tooth must be monitored because it no longer wears normally.
Owners may need to rinse or inspect accessible areas and provide softer forage temporarily. Body weight and chewing efficiency should be followed, with a balanced soaked ration used if chronic disease limits fibre processing. Early pockets can stabilise, but advanced periodontitis often needs lifelong surveillance. Comfort, function and control of feed packing are more realistic outcomes than restoring destroyed bone.
