Osteoarthritis is progressive deterioration of an entire synovial joint, involving articular cartilage, underlying bone, synovial lining, capsule and supporting soft tissues. It can follow repeated athletic loading, joint instability, fracture, osteochondrosis, infection or a single traumatic injury. Once cartilage loses its smooth resilient structure, inflammation and abnormal force reinforce one another, producing pain and further degeneration.
Signs depend on the joint and workload. Lameness may be most apparent when work begins, improve after warming up, then worsen with fatigue. Reduced range of motion, joint effusion, firm capsular thickening, crepitus and reluctance to perform particular movements can develop. Some low-motion joints create substantial pain before visible swelling, while structural joint change can be advanced in a horse with modest clinical signs.
Osteoarthritis is not one uniform “old horse” condition. Young athletes can develop it after instability or injury, while an elderly horse may remain comfortable despite extensive remodelling. Pain arises from inflamed synovium, capsule, subchondral bone and supporting tissues rather than cartilage alone. A suddenly hot, swollen and severely painful joint is a different pattern that can reflect rapidly destructive infection.
Established cartilage loss cannot be restored to normal. Treatment aims to reduce inflammation, improve mechanics, maintain muscle and preserve a comfortable level of function. Prognosis depends on the joint, severity, stability, intended work and response. Some horses continue at a modified level for years; high-motion joint disease with marked cartilage loss can end athletic use despite intensive care.
Compensatory loading can transfer strain to the opposite limb, back or adjacent joints long before the original joint becomes visibly enlarged. High-motion joints often remain performance-limiting because movement repeatedly stresses painful tissues, whereas some low-motion joints become less painful as mobility diminishes. This difference helps explain why similar degrees of structural change can produce very different functional outcomes.
A lameness examination records onset, surface, direction, warm-up effect, workload and prior joint injury or injection. The veterinarian inspects symmetry, effusion and muscle loss, palpates and flexes joints, then observes the horse under safe controlled conditions. Diagnostic nerve or intra-articular analgesia localises pain; improvement after a block is interpreted with the known spread of anaesthetic.
Radiographs assess joint space, osteophytes, subchondral sclerosis, lysis and old fragments. Early cartilage disease may be radiographically silent, so ultrasound, nuclear scintigraphy, CT, MRI or arthroscopy is selected according to location and unanswered question. Synovial-fluid analysis is essential when infection is possible and should precede corticosteroid medication.
Repeat examination and imaging monitor progression, but treatment decisions should follow comfort and function as well as pictures. Owners can track stiffness, willingness to turn, joint swelling and recovery after work. Sudden marked lameness, fever, a penetrating wound near a joint or rapid effusion requires urgent assessment rather than assuming a flare of chronic arthritis.
Diagnostic joint analgesia is interpreted alongside imaging because blocking neighbouring structures can blur localisation.
Management starts by correcting the initiating problem where possible: remove an osteochondral fragment, stabilise injury, treat infection or improve hoof balance. Work is modified to reduce damaging load without abandoning all movement. Controlled regular exercise maintains cartilage nutrition, muscle and range of motion; acute painful flares may require a short reduction before rehabilitation resumes.
Systemic NSAIDs provide analgesia but require attention to hydration, gastrointestinal and kidney risk. Intra-articular corticosteroid, hyaluronic acid or selected biologic products may reduce inflammation when the joint has been accurately localised and sepsis excluded. Sterile technique and appropriate intervals are essential. Oral or injectable “joint supplements” vary widely in evidence and should not replace mechanical correction.
Farriery can shorten breakover, improve balance or alter load for a specific joint. Weight control, suitable footing, warm-up and training variation reduce cumulative stress. Arthroscopic debridement treats selected fragments or focal lesions; low-motion painful joints may ultimately be fused medically or surgically to abolish movement. Follow-up evaluates lameness and workload response rather than repeating injections automatically. The goal is sustained comfort at a realistic job, with retirement considered when pain control depends on unsafe medication or continuing work accelerates damage.
