Neonatal septicaemia is bloodstream infection with a harmful systemic inflammatory response in a foal, usually during the first week of life. It can begin in the uterus through placentitis or ascend after birth through the gastrointestinal tract, umbilicus, lungs or wounds. Failure of passive transfer, prematurity, dystocia, a sick mare and an unsanitary environment increase risk, but a well-managed foal can still become septic.
Early signs are easily mistaken for tiredness: reduced nursing, sleeping more, weakness, poor weight gain, abnormal temperature and separation from the mare. Progression can produce rapid or slow heart rate, injected or pale gums, petechiae, prolonged capillary refill, diarrhoea, respiratory distress, swollen joints, lameness, seizures, shock and recumbency. Foals can be seriously septic without fever.
Gram-negative and gram-positive bacteria are involved, sometimes together; E. coli, Klebsiella, Actinobacillus, Streptococcus, Staphylococcus and Enterococcus are among recognised isolates. Organisms seed joints, growth plates, lungs, meninges, eyes and kidneys, so apparent improvement in alertness does not exclude a focal infection.
This is a time-critical emergency. Blood culture is important, but treatment begins before results because deterioration can be measured in hours. Prognosis varies with prematurity, organisms, shock, number of infected sites and how soon effective therapy starts. Survivors require follow-up for septic arthritis, physitis, pneumonia and neurological injury that may appear after the bloodstream infection is controlled. Prevention combines good foaling hygiene, rapid correction of passive-transfer failure and close observation of normal neonatal milestones.
Neonatal immune and circulatory responses differ from those of adult horses, so hypothermia or a low white-cell count can signal overwhelming infection rather than reassurance. Bacteria may seed a growth plate or joint before visible swelling develops. Once infection localises in bone, drug penetration and future athletic soundness become additional concerns.
Any foal that is slow to stand, stops nursing, becomes lame, has diarrhoea or simply appears quieter receives a complete neonatal examination. Temperature, heart and respiratory rates, perfusion, blood pressure, glucose, oxygenation, weight, umbilicus, lungs, joints and neurological status are assessed repeatedly because trends matter more than one normal value.
Blood is collected aseptically for culture before antibiotics when this causes no meaningful delay. Complete blood count, chemistry, lactate, glucose, blood gas, fibrinogen or serum amyloid A and serum IgG evaluate inflammation, organ function and passive transfer. Ultrasound examines lungs, abdomen and umbilical remnants. Radiographs and sterile joint-fluid samples investigate lameness; cerebrospinal fluid is considered for neurological signs.
Placenta, mare and foaling history provide important clues. Sepsis scores can organise findings but neither confirm nor exclude infection by themselves. Culture-negative sepsis is possible after prior antibiotics or intermittent bacteraemia. Frequent reassessment determines response and reveals new focal sites.
Urine output is measured closely because shock and nephrotoxic drugs can compound renal injury.
Broad-spectrum intravenous antimicrobials are started immediately after cultures and adjusted to organism, susceptibility, local resistance and infection site. Initial combinations cover gram-negative and gram-positive bacteria; aminoglycosides require renal and drug-level monitoring. Treatment duration is longer for joints, bone, meninges or deep umbilical infection than for uncomplicated bacteraemia.
Intensive support restores circulation and oxygen delivery with carefully titrated fluids, plasma, vasopressors, oxygen or ventilation. Glucose, nutrition, temperature and acid-base disturbances are managed continuously. Failure of passive transfer is corrected with intravenous plasma. Anti-inflammatory treatment is balanced against kidney and gastrointestinal risk in a dehydrated foal.
Septic joints are lavaged promptly and infected umbilical remnants may need surgical removal. Pneumonia, diarrhoea, seizures and neonatal encephalopathy receive specific care. The foal nurses from the mare when safe or receives measured enteral or parenteral nutrition; prolonged fasting weakens recovery. Barrier nursing and hygiene limit spread of enteric or resistant organisms. Discharge follows stable nursing, normalising inflammatory markers and control of every focal infection, with scheduled soundness and growth-plate checks rather than assuming that survival alone equals complete recovery.
Inflammatory markers are followed after appetite returns because focal infection may otherwise be missed.
