Leptospirosis is infection with pathogenic spiral-shaped Leptospira bacteria. Organisms shed in the urine of infected wildlife, livestock or other animals survive best in wet environments and enter a horse through mucous membranes or damaged skin. After a bloodstream phase they can localise in the kidneys, placenta, fetus and eye. Human infection is possible, so urine and abortion material require hygienic handling.
Many exposed horses remain clinically normal. Recognised disease includes fever with acute kidney injury, late-term abortion or birth of a weak foal, and equine recurrent uveitis. Affected kidneys can cause depression, reduced appetite, increased or decreased urination, abnormal urine and rising waste products. Leptospiral placentitis may produce abortion without prolonged illness in the mare; organisms can then be shed in urine for weeks or months. Uveitis may occur later and recur after the initial systemic infection has passed.
A positive blood antibody titre does not by itself prove that Leptospira caused current disease because exposure can be common in some regions and vaccination also affects results. Conversely, antibody can be low in chronic localised eye disease. Diagnosis must connect the organism or a meaningful antibody response with compatible kidney, reproductive or ocular findings.
Leptospirosis is not one uniform syndrome and should not receive a single generic treatment. Acute bacteraemia may respond to systemic antimicrobials, whereas established recurrent uveitis is driven by persistent local infection and inflammation requiring specialist ophthalmic management. Prognosis is generally good for treated systemic disease but depends on renal damage, pregnancy loss and the extent of irreversible injury inside the eye.
Leptospira organisms are shed in urine and persist in wet environments, entering through mucosa or damaged skin. Equine infection is often subclinical but is associated with abortion, placentitis, neonatal illness and recurrent uveitis. The strain ecology differs by region. People can be infected from urine or reproductive fluids, so gloves and hygiene matter during suspected cases.
Risk assessment covers standing water, flooding, rodents, wildlife, livestock contact, recent abortion and exposure to urine. A mare that aborts should be separated while the fetus and placenta are secured for veterinary examination; handlers wear gloves and protect eyes and broken skin. The placenta should be submitted intact where possible rather than discarded.
For abortion, fluorescent antibody testing, immunohistochemistry or PCR on placenta, umbilical cord, fetal kidney and liver provides stronger evidence than maternal serology alone. PCR on blood is most useful early in acute infection, while urine PCR may detect renal shedding. Paired microscopic agglutination titres can show a marked rise, but cross-reactions and vaccination history require interpretation.
Kidney cases receive blood chemistry, urinalysis and renal imaging as indicated. Ocular examination includes fluorescein staining, intraocular pressure and detailed assessment of the anterior and posterior eye; ocular-fluid PCR or antibody testing is reserved for selected cases because sampling is invasive. A painful, cloudy or light-sensitive eye is an emergency and should not wait for laboratory confirmation before inflammation is controlled.
Acute leptospirosis is treated with a veterinarian-selected systemic antimicrobial such as penicillin or a tetracycline, together with fluids and electrolyte care for kidney injury. Drug choice accounts for renal function, pregnancy and local evidence. Treatment aims to resolve bacteraemia and reduce renal shedding, but follow-up urine testing may be appropriate because clinical improvement does not guarantee immediate clearance.
Leptospira-associated abortion also requires examination of the mare for retained membranes, uterine infection and systemic illness. Contaminated bedding and fluids are handled as a zoonotic risk, with gloves, covered wounds and effective cleaning. Other pregnant mares are managed according to exposure, regional disease and veterinary advice rather than automatically given an unverified regimen.
Established recurrent uveitis needs prompt anti-inflammatory and pupil-dilating eye medication; systemic antibiotics alone do not stop recurrent immune-mediated damage. Refractory Leptospira-associated cases may be candidates for intravitreal medication or vitrectomy by an equine ophthalmologist. Prevention includes rodent control, reducing contamination of feed and water, and vaccination where a licensed product matches regional serovar risk. Prognosis is guarded for vision once repeated uveitis has caused synechiae, cataract, glaucoma or retinal injury.
