The horse’s large colon is only loosely attached within the abdomen, allowing it to move into an abnormal position or rotate. A displacement can obstruct the passage of gas and ingesta and may cause intermittent or progressive colic. Volvulus is a more dangerous rotation in which the colon twists around its attachment, compressing veins and arteries. Loss of blood supply rapidly causes swelling, tissue death, endotoxin absorption and shock.
Large-colon displacement may produce moderate recurrent pain, reduced manure, abdominal enlargement and an incomplete response to analgesia. Some right dorsal displacements trap the colon between the caecum and body wall, while nephrosplenic entrapment places part of the colon over the nephrosplenic ligament. A complete volvulus generally causes severe, relentless pain, sweating, rapid heart rate, congested gums and deterioration despite medication. Individual presentations vary, and an early volvulus may not yet display every sign of shock.
Post-foaling mares are at increased risk of large-colon volvulus, probably because sudden abdominal space changes allow greater movement. Large meals, gas production, recent management change and previous displacement may also contribute, but many cases have no single preventable trigger. The condition must not be confused with a simple impaction merely because manure has decreased.
Tissue survival is time-dependent. A non-strangulating displacement may sometimes resolve medically, whereas a volvulus requires urgent surgery to restore circulation. Even after correction, damaged bowel can release inflammatory products and cause postoperative ileus, endotoxaemia, thrombosis or laminitis. A horse may therefore remain critically ill after the colon is repositioned, and intensive postoperative monitoring is part of the emergency rather than optional aftercare. Prognosis is strongly influenced by the duration and degree of rotation, cardiovascular damage and the speed with which the horse reaches surgery.
There is no dependable screening test for future displacement or volvulus. Early detection begins with recognising colic that is persistent, recurrent or disproportionate to apparently mild findings. Owners should note the onset of pain, manure output, abdominal distension, response to medication and recent foaling or previous displacement. Pain returning quickly after analgesia is a major reason for immediate reassessment rather than repeated home dosing.
Veterinary examination may identify abnormal colon position or severe gas distension on rectal palpation. Ultrasound can show displaced colon, thickened bowel wall or abnormal vessels, although not every region is visible. Heart rate, gum colour, lactate, blood tests and abdominal-fluid analysis help assess perfusion and tissue injury. Nasogastric reflux is often limited because the lesion is in the large bowel, so absence of reflux does not make the condition safe.
Serial examinations are valuable in equivocal cases, but they must not delay referral when pain persists or cardiovascular findings worsen. A rapidly rising heart rate, unrelenting pain, dark or congested gums, increasing lactate and abnormal abdominal fluid support urgent exploratory surgery. Definitive identification of some displacements occurs only during the operation.
Initial care focuses on pain control, intravenous fluids and rapid assessment of whether the colon’s blood supply is threatened. A veterinarian may attempt medical correction for carefully selected non-strangulating displacements. Management can include withholding feed, fluid therapy and specific movement or rolling procedures performed under controlled veterinary conditions. These techniques are not suitable for a horse with suspected volvulus, severe pain or cardiovascular compromise.
Persistent displacement, severe distension and volvulus require abdominal surgery. The colon is decompressed and returned to its normal position; the surgeon evaluates colour, motility and blood flow to determine whether tissue can recover. Irreversibly damaged bowel may require resection, although the size and anatomy of the large colon can limit what is possible. Delays reduce the chance of survival because venous congestion progresses to arterial obstruction and systemic shock.
Postoperative treatment may include fluids, analgesia, antimicrobial therapy when indicated, anti-endotoxin measures and careful nutritional reintroduction. The horse is monitored for ileus, recurrent pain, incision problems, diarrhoea and laminitis. Exercise resumes through a staged rehabilitation plan after abdominal healing. Some horses experience recurrent displacement, so diet, turnout and any individual risk factors are reviewed, but no management programme can guarantee prevention. Prognosis is fair to good for an uncomplicated displacement and guarded when volvulus has caused extensive bowel and circulatory injury.
