Laminitis is failure of the interlocking laminae that suspend the pedal bone within the hoof capsule. Injury weakens the attachment between bone and hoof wall; in severe cases the pedal bone rotates or sinks under body weight and deep digital flexor tendon forces. This is a medical emergency because structural damage can begin before obvious pain and become irreversible quickly.
Major pathways include insulin-associated laminitis in horses with insulin dysregulation, sepsis-associated disease after colitis, retained placenta or severe infection, and supporting-limb laminitis when another painful limb forces sustained overload. Grain overload and some toxic exposures can also trigger disease. These mechanisms differ, so the cause must be identified rather than treating every case as excess grass.
Typical signs are reluctance to walk or turn, a shortened pottery gait, repeated weight shifting, heat in the feet and strong digital pulses. Horses with forefoot pain may lean back to unload the toes. One or all four feet can be involved, and supporting-limb disease may begin in a single overloaded foot. Chronic changes include divergent hoof rings, a stretched white line, dropped sole, widened toe and recurrent abscesses.
Pain intensity does not perfectly indicate pedal-bone stability. Radiographic rotation or sinking can progress despite temporary comfort from analgesia. Prognosis depends on the inciting disease, number of feet, depth of structural failure, sole thickness and response to mechanical support. Early recognition and coordinated veterinary-farriery care offer the best chance of preserving a comfortable, functional foot.
Failure of the lamellar attachment suspending the distal phalanx can permit rotation or sinking within the hoof capsule. A horse may shift weight, adopt a rocked-back stance, resist turning or show bounding digital pulses before radiographic displacement occurs. Supporting-limb laminitis and sepsis-associated disease can affect feet asymmetrically, so the classic two-front-foot posture is not required.
At-risk horses require proactive foot monitoring. During severe colitis, endotoxaemia, retained placenta or major opposite-limb lameness, digital pulses, hoof temperature, stance and willingness to turn should be checked repeatedly. Horses with obesity, a cresty neck, previous pasture-associated laminitis, PPID or suspected insulin dysregulation need endocrine assessment even when currently sound.
Veterinary examination evaluates gait on a safe surface, response to turning, hoof-tester sensitivity and digital pulses. Forced exercise is avoided when laminitis is possible. Baseline lateral and dorsopalmar radiographs document pedal-bone position, sole depth and hoof-capsule alignment; repeat images track displacement and guide trimming. Radiopaque markers on the hoof wall improve measurements.
Blood testing targets the cause, including insulin assessment, ACTH for PPID and evaluation for systemic inflammation or organ dysfunction. A normal early radiograph does not exclude laminar injury because displacement may not yet have occurred. Owners should seek immediate care for a new bounding pulse, characteristic stance or reluctance to turn, rather than waiting for severe lameness or visible hoof distortion.
The inciting disease and the foot must be treated simultaneously. Developmental sepsis-associated cases benefit from continuous distal-limb cryotherapy started as early as possible and maintained at an effective low temperature under veterinary supervision. Acute insulin-associated cases require immediate control of dietary non-structural carbohydrate and hyperinsulinaemia. Supporting-limb cases need aggressive relief of the primary limb and load distribution.
The horse is confined on deep, conforming bedding and is not walked to “improve circulation”. Analgesia and anti-inflammatory medication are balanced against hydration and organ function. Frog and sole support, impression material, boots, casts or temporary mechanics redistribute load away from damaged dorsal laminae. Shoe removal or trimming is planned from radiographs; indiscriminate pulling or aggressive toe removal can worsen instability.
Serial radiographs and comfort guide staged farriery as the hoof grows. Chronic cases may need realignment trimming, therapeutic shoes or boots, abscess management and months of restricted activity. Feed is weighed and analysed for horses with insulin dysregulation, while abrupt starvation is avoided. Prognosis is reassessed honestly if pain remains uncontrollable, the pedal bone sinks extensively or penetrates the sole. Humane euthanasia is appropriate when a comfortable life cannot be restored.
