Insulin dysregulation describes abnormal control of insulin and glucose after feeding or during changing physiological demand. It can include an excessive insulin response to oral carbohydrate, persistently high resting insulin, tissue insulin resistance and reduced liver clearance of insulin. The clinically important consequence is hyperinsulinaemia-associated laminitis, not diabetes mellitus; most affected horses maintain normal or near-normal blood glucose.
Insulin dysregulation is the core abnormality in equine metabolic syndrome and can also occur with PPID, pregnancy, obesity or other physiological stress. Some affected equids are lean and have no obvious neck crest. Others show regional or general fat accumulation, recurrent pasture-associated foot soreness, widened white lines or hoof rings indicating previous laminar stress. Insulin status can change with diet, season, body fat, fitness, pain and concurrent illness.
The term is more precise than assuming every easy keeper is “insulin resistant”. A horse may maintain ordinary insulin between meals yet produce a dangerous post-meal surge, while pain or physiological stress can temporarily intensify the response. Clinically important disease reflects the magnitude and duration of hyperinsulinaemia and the vulnerability of the hoof lamellae, not body shape alone.
There is no visual threshold at which pasture becomes safe, and grass sugar varies with plant species, weather and time. Management aims to keep post-meal insulin below a laminitis-promoting range while maintaining adequate fibre, protein, minerals and healthy body condition. Insulin dysregulation can improve markedly but predisposition often remains, requiring lifelong monitoring during pasture changes, ageing and onset of PPID.
Persistent insulin exposure changes blood-vessel signalling and cellular behaviour within the hoof lamellae. Damage can develop without the severe systemic illness seen in grain-overload laminitis, allowing repeated mild episodes to pass as foot soreness or reluctance on firm ground. Pancreatic compensation often keeps glucose stable even while insulin concentrations place the feet at continuing risk.
A resting insulin sample is a practical first screen when collected under a defined feeding or short-fasting protocol. A clearly high result is meaningful, but a low value does not exclude an excessive meal response. Dynamic oral sugar or oral glucose testing measures insulin after a standard challenge and is generally more sensitive for postprandial dysregulation. Intravenous tests are available for specialist investigation.
Exact dose, sample time, fasting conditions and assay-specific cut-offs must be recorded; results from different protocols are not interchangeable. Testing during severe pain, transport stress or acute illness can mislead. Blood glucose alone is not a useful screen because diabetes is uncommon. Older horses should also have seasonally interpreted PPID testing, and the feet should be examined and radiographed when past laminitis is suspected.
Follow-up testing after dietary change, weight loss or medication evaluates the current risk rather than conferring permanent clearance. A horse with active laminitis is stabilised before a carbohydrate challenge. Owners should record the actual forage batch, grazing exposure and body measurements so a change in insulin result can be connected to management rather than guessed.
Treatment removes large insulin-provoking meals while preserving safe nutrition. Low-non-structural-carbohydrate hay is selected by analysis, weighed and distributed slowly across the day. Grain, sweet feeds and sugary treats are stopped. Pasture access is restricted or eliminated until insulin response and hoof stability improve; a grazing muzzle reduces intake but does not make every pasture safe.
Weight loss is gradual for obese horses, with a balanced ration supplying protein, vitamins and minerals. Sound horses benefit from regular exercise because muscle activity improves metabolic control, but exercise is contraindicated during active laminitis. Hoof support and radiographic farriery are addressed even when lameness is subtle. PPID is treated concurrently when present.
If rigorous diet and feasible exercise do not control insulin, a veterinarian may consider medication. Metformin has inconsistent systemic absorption but can reduce meal response in selected horses when timed before feeding. SGLT2 inhibitors can lower insulin in severe cases yet may cause substantial hypertriglyceridaemia, so specialist oversight and serial lipid monitoring are necessary. Supplements marketed for insulin sensitivity should not replace measured forage and repeat testing. Management is adjusted using post-meal insulin, body condition and foot health together.
