Headshaking syndrome describes repeated, involuntary vertical, horizontal or rotary head movements that cannot be explained by normal behaviour. Many affected horses also snort, rub the nose, strike at the muzzle or act as if an insect has entered a nostril. Signs often worsen during exercise, bright light, wind or particular seasons and can make riding dangerous.
The common primary form is trigeminal-mediated headshaking, in which the trigeminal sensory pathway has an abnormally low firing threshold. Ordinary stimulation of the face and nasal passages is experienced as sharp, tingling or electric discomfort. Some cases are photic and triggered by bright light, but this is not simply an allergy or a training problem. Secondary headshaking results from a separate painful disorder such as ocular disease, dental disease, ear pathology, sinusitis, upper-airway lesions, neck pain, tack pressure or occasionally neurological disease.
Diagnosis therefore requires exclusion of treatable causes before the primary label is applied. Repetitive nodding only under saddle may also reflect lameness, rider or tack effects, learned behaviour or conflict, whereas trigeminal signs often include sudden nasal irritation and persist with different handlers. The pattern can remit in winter and recur each spring.
The syndrome is not contagious. Severity ranges from an occasional manageable flick to violent movements that prevent turnout, eating or safe handling. No single treatment works for every horse, and spontaneous remission complicates assessment. Prognosis is fair for mild cases controlled by a nose net or environmental change and guarded for severe persistent disease. A horse that might strike its own face, fall or injure a rider should be withdrawn from work while investigated.
The apparent trigger can change with season and environment, and a horse may be normal at rest yet violently affected at a particular gait outdoors. Trigeminal-mediated disease resembles neuropathic pain more than deliberate evasion. This distinction prevents escalating bits, nosebands or punishment, which may increase distress and create additional oral or poll pain.
The veterinarian records the exact movement, nasal behaviours, season, light, wind, exercise intensity, location, tack and rider. Owner video from natural episodes is valuable. A complete physical, neurological, orthopaedic, dental and ophthalmic examination looks for pain that can mimic primary headshaking; ears, sinuses, nostrils, guttural pouches and upper airway are assessed as indicated.
Controlled exercise observations compare indoor and outdoor work, bright and dim conditions, with and without tack and, when safe, a properly fitted nose net. Endoscopy and skull or dental imaging are selected from findings rather than performed as a fixed panel. Local anaesthetic blocks may help localise dental or facial pain, while specialist trigeminal nerve blocks have interpretive limitations and require expertise.
There is no routine blood test or scan that proves trigeminal-mediated disease. Diagnosis is made from a characteristic pattern after reasonable exclusion of secondary causes. A diary with videos and a consistent severity scale establishes baseline and measures treatment. Violent episodes, weight loss, eye pain, unilateral discharge, neurological deficits or abrupt onset demand urgent reassessment rather than assuming recurrence.
Any secondary cause is treated directly: diseased teeth, painful eyes, sinus or ear lesions, lameness and ill-fitting tack are not managed as idiopathic headshaking. For trigeminal-mediated disease, a correctly fitted mesh nose net is a useful low-risk first trial and helps a substantial subset of horses. UV-blocking masks, indoor exercise or avoiding bright, windy periods may help photic or seasonal cases without eliminating turnout needs.
Medical options such as cyproheptadine, carbamazepine or gabapentin have inconsistent responses and potential sedation or competition restrictions; they require veterinary supervision and a measured trial. Percutaneous electrical nerve stimulation of the infraorbital nerves can provide temporary improvement in some horses and is usually delivered as a series by an experienced clinician. Surgical or ablative procedures carry significant adverse effects and are reserved for carefully selected severe cases.
Every trial uses the same activity and objective video score so seasonal change is not mistaken for success. Riding stops if movements impair control or balance. Management includes facial protection from self-trauma and humane welfare review when distress persists at rest. Supplements and allergy treatment should not be continued solely on anecdote. Long-term plans are individual, with periodic attempts to reduce medication during naturally quiet periods and rapid reassessment if the pattern changes.
