Guttural pouch empyema is accumulation of infected purulent material within one or both guttural pouches, the paired air-filled extensions of the auditory tubes unique to equids. It most often follows an upper respiratory bacterial infection or rupture of retropharyngeal lymph-node abscesses associated with strangles. Streptococcus equi must therefore be considered until testing shows otherwise.
Discharge is usually thick, cream or yellow and may appear intermittently from one or both nostrils, particularly when the horse lowers its head. Other signs include fever, poor appetite, painful swelling behind the jaw, stiff head carriage, cough, noisy breathing and difficulty swallowing. Large accumulations can compress the pharynx and threaten the airway. Chronic pus loses water and forms firm oval concretions called chondroids.
Chondroids and retained exudate allow bacteria to persist after the original illness appears resolved. An outwardly healthy carrier can intermittently shed S. equi and initiate new strangles outbreaks months later. Empyema is therefore not simply ordinary nasal discharge, and closure of an external abscess does not prove the guttural pouches are clear.
Other causes of pouch discharge or dysfunction include mycosis, cysts, tumours and foreign material. The likely prognosis is good when liquid exudate is recognised and removed early. Dense chondroid burdens, retropharyngeal abscessation or severe airway compression make treatment more involved. Ongoing dysphagia also raises concern for nerve injury or another pouch disorder and needs specific investigation rather than repeated blind antibiotic courses.
Pus can dehydrate into firm chondroids that remain for months or years and intermittently release bacteria into the pharynx. Strangles is an important cause, but other upper-airway infections can be involved. Swelling behind the jaw, nasal discharge and noisy swallowing vary with pouch filling, and a clinically normal carrier may still introduce S. equi to a new group.
A horse with compatible discharge or recent strangles should be isolated while the cause is investigated. The veterinarian examines the upper airway and both guttural pouches endoscopically, determining whether disease is unilateral, bilateral, liquid or organised into chondroids. Radiographs may demonstrate a pouch fluid line, chondroids or a retropharyngeal mass.
Purulent material is collected directly for cytology, bacterial culture and PCR for S. equi. A superficial nasal swab is less representative and intermittent drainage can produce a false-negative result. Endoscopic guttural-pouch lavage after clinical recovery is also the preferred way to assess a potential strangles carrier. Testing strategy and release timing should follow outbreak risk and veterinary guidance.
Breathing noise, marked pharyngeal compression, distressed swallowing or respiratory effort requires emergency airway assessment. Examination should distinguish empyema from fungal plaques and vascular lesions because manipulation of guttural-pouch mycosis can provoke fatal haemorrhage. Repeat endoscopy documents removal of material and mucosal recovery; disappearance of visible nasal discharge alone is not an adequate clearance test.
Repeated lavage of the affected guttural pouch is the foundation of treatment because systemic antibiotics alone rarely remove retained pus. Saline is delivered and recovered through an endoscopically placed catheter with care to prevent aspiration. Local antimicrobial preparations and systemic treatment may be used when active infection, fever, surrounding cellulitis or a confirmed organism warrants them.
Small numbers of chondroids can be retrieved with endoscopic baskets, snares or forceps. Large masses, extensive organised material or an unresolved retropharyngeal abscess may need endoscopic fragmentation or surgical drainage. A horse with pharyngeal compression and impending obstruction may require a temporary tracheotomy before pouch procedures continue. Feeding texture and route are adjusted if swallowing is unsafe.
S. equi biosecurity remains in place until the horse is clinically recovered and veterinarian-directed testing confirms clearance. Equipment used for lavage and nasal secretions is contained and disinfected, and handlers use dedicated clothing and hand hygiene. Follow-up endoscopy is important because a few retained chondroids can maintain the carrier state. Prognosis is favourable with complete drainage; treatment becomes longer when concretions are numerous or deeper abscessation persists. Any continuing dysphagia after the pouch is clean should be assessed for cranial-nerve damage rather than assumed to resolve automatically.
