A fracture is a partial or complete break in bone. In horses it ranges from a small stress fracture or non-displaced crack to catastrophic fragmentation of a weight-bearing limb. Outcome depends on bone, location, displacement, skin penetration, joint involvement, horse size and whether further damage is prevented immediately.
Acute signs include sudden severe or non-weight-bearing lameness, swelling, abnormal angle, instability or a heard crack. Stress fractures can be subtler, causing intermittent lameness that worsens with exercise before complete failure. An open fracture communicates with a wound and is contaminated even when only a tiny puncture overlies the break. Fractures entering a joint threaten both joint congruity and infection.
Horses must bear substantial weight on four relatively slender limbs and cannot safely remain recumbent for long. A badly unstable limb can sever vessels or tear through skin during one unnecessary step. The opposite supporting limb is also at risk of laminitis during prolonged recovery. Nevertheless, not every fracture is hopeless: selected pedal-bone, splint-bone, sesamoid, skull and non-displaced long-bone fractures can heal with appropriate care, and modern fixation allows some severe injuries to be repaired.
Prognosis cannot be determined from lameness alone or before stabilisation and imaging. A calm horse with intact blood supply and correctly splinted limb has more options than one allowed to walk onto a float unsupported. Emergency priorities are human safety, preventing displacement and arranging expert assessment—not testing whether the horse can bear weight.
Equine fractures range from stable incomplete stress lines to displaced open injuries that threaten skin and blood supply. Continued weight bearing can convert a repairable crack into a catastrophic break. Outcome depends on bone, configuration, contamination, horse size and opposite-limb support; the old assumption that every equine fracture is untreatable is inaccurate.
A suspected fracture is treated as unstable until proved otherwise. The horse is kept still and calm; if recumbent, the limb is assessed before any attempt to make it stand. The veterinarian checks skin, pulses, sensation, limb alignment and other injuries while limiting manipulation. An unstable limb is padded and splinted according to fracture level before radiographs or transport.
Radiographs in at least two views define most fractures, with additional oblique views, CT, MRI or nuclear scintigraphy for complex, skull or occult stress injuries. Ultrasound can identify some cortical breaks and soft-tissue damage. Repeat imaging may be required when an early stress fracture is suspected but not initially visible.
Any nearby wound is investigated for communication with bone, joint or tendon sheath; sterile synovial sampling and contrast studies may be necessary. Blood tests assess haemorrhage and systemic status rather than confirm the break. Sudden severe lameness in a racehorse or after a kick should not be lunged for localisation. Transport occurs only after the limb and trailer setup are planned by the attending veterinarian.
Emergency treatment uses sedation, analgesia, wound covering and correctly positioned padding and splints. Splint choice depends on the level and displacement because the wrong configuration can increase leverage. Open bone is covered with a sterile dressing and is not scrubbed aggressively in the field. Transport is slow in a prepared vehicle, with loading direction and support chosen for the injured limb.
Definitive options include stall confinement for stable non-displaced fractures, casts or transfixation devices, and surgical fixation with screws, plates or other implants. Open fractures require debridement, lavage and targeted antimicrobial therapy. Joint reconstruction must restore alignment as closely as possible. Selected splint-bone fragments can be removed after stability is assessed.
Recovery involves radiographic monitoring, cast and pressure-sore care, controlled reloading and prevention of supporting-limb laminitis. Nutrition supports healing without excessive weight gain. Complications include implant failure, infection, delayed union and catastrophic breakdown during anaesthetic recovery; specialist centres plan assisted recovery for high-risk repairs. Humane euthanasia is appropriate when stable fixation, blood supply or a pain-free outcome is not achievable. The decision follows precise fracture assessment, not a blanket rule that every equine fracture is untreatable.
