Equine sarcoids are the most common skin tumour of horses, donkeys and mules. They are locally invasive fibroblastic growths associated with bovine papillomavirus types 1 and 2. Sarcoids do not usually spread to internal organs, but they can infiltrate surrounding skin, recur aggressively after disturbance and seriously interfere with tack, vision, limb movement or wound healing.
Six clinical patterns are recognised: occult flat hairless patches; verrucose scaly or warty plaques; small firm nodules; fibroblastic fleshy ulcerated masses; mixed lesions; and aggressive malevolent cords or nodules extending through skin. One horse may have several types. Common sites include face and eyelids, groin, sheath, axilla, limbs and previous wounds.
Appearance can resemble a wart, proud flesh, fungal patch or another tumour. Trauma, rubbing, casual biopsy or incomplete excision may stimulate rapid growth, yet avoiding biopsy indefinitely can leave a dangerous misdiagnosis. Sampling strategy should be planned with the treatment that will follow. Flies may transfer viral material between wounds, but sarcoids are not managed like a simple contagious respiratory outbreak.
No single treatment is best for every lesion. Site, type, size, number, prior treatment and available expertise determine outcome. Small stable occult lesions can sometimes be monitored, while periocular or rapidly enlarging fibroblastic lesions need early specialist planning. Recurrence after surgery alone is common because microscopic tumour and virus-associated change extend beyond visible margins. Prognosis is good for control of many small lesions and guarded for multiple, recurrent, periocular or malevolent disease.
Sarcoids are locally invasive fibroblastic skin tumours associated with bovine papillomavirus. Occult, verrucous, nodular, fibroblastic, mixed and malevolent forms behave differently, and trauma can provoke rapid growth. They do not metastasise to distant organs, yet their location around eyes, girth or limbs can make local control difficult and recurrence common.
Every lesion is mapped, measured and photographed, including scars and areas under tack. The veterinarian records growth rate, ulceration, previous trauma and prior treatments. Multiple characteristic lesions can support a clinical diagnosis, but atypical solitary masses require differentials including squamous cell carcinoma, melanoma, habronemiasis, granulation tissue and infectious granuloma.
Biopsy provides histological confirmation but can activate some sarcoids. It is undertaken only when the result will change management and a definitive treatment plan is ready. Excisional biopsy may be appropriate for a small accessible lesion with adequate margins; superficial snipping or owner-applied caustic treatment is not. Ultrasound defines depth in selected nodules, and ocular examination is essential near the eyelid.
Serial standardised photographs are reasonable for deliberately monitored stable occult lesions. Any growth, surface change, tack interference or new satellite nodule triggers reassessment. There is no validated blood or screening DNA test for an unaffected horse. A pre-purchase examination should describe site and type without promising that a quiet lesion will remain unchanged.
Treatment is customised and commonly combines methods. Wide surgical or laser excision can remove accessible masses, but surgery alone has a substantial recurrence risk. Local chemotherapy with cisplatin or other agents, electrochemotherapy, cryotherapy, topical immune-modulating medication and ligation of selected pedunculated lesions can improve control. Periocular lesions may respond to carefully administered BCG immunotherapy or brachytherapy in specialist centres.
Caustic proprietary creams can cause marked swelling, pain and tissue loss and must be used only with veterinary diagnosis and monitoring, especially near eyes or major vessels. Incomplete home cutting, tying or burning can convert a quiet lesion into aggressive fibroblastic disease. Large masses may require staged debulking plus adjunctive therapy and meticulous wound management.
Fly control and protection from rubbing reduce ulceration and possible mechanical spread. Treated sites are monitored for years because recurrence can begin beneath an apparently healed scar. A small stable occult lesion in a low-risk location may be photographed rather than disturbed when treatment risk exceeds benefit. Prognosis is most favourable for first-time, small, accessible lesions treated with an appropriate combined protocol; repeated recurrence narrows options and increases cost and welfare burden.
