Equine piroplasmosis is a blood-borne protozoal disease caused by Theileria equi or Babesia caballi. Competent ticks acquire the parasites from an infected equid and transmit them during later feeding. Infection also spreads through reused needles, contaminated intravenous equipment, blood products and other practices that transfer blood. Mares can occasionally infect foals before birth.
Parasites invade red blood cells, producing fever, anaemia and red-cell destruction. Acute disease may cause marked depression, poor appetite, weakness, rapid heart and respiratory rates, pale or yellow mucous membranes, dark urine, small dry manure balls, mild colic and limb swelling. Peracute cases can die, while chronic infections may show only poor performance, weight loss or intermittent fever. Horses that survive can remain long-term or lifelong carriers even when they appear healthy.
The disease is endemic in many tropical and temperate regions but is exotic or tightly controlled in others, including Australia. Imported and internationally travelled horses therefore have important testing and reporting requirements. A carrier can introduce parasites into an area with suitable ticks or transmit them through blood-contaminated procedures.
Clinical signs overlap with EIA, leptospirosis, immune-mediated haemolysis and other febrile diseases. Identification of anaemia alone is not diagnostic. There is no vaccine. Prognosis for the individual varies with parasite species, prior exposure and severity, while the population consequence depends on national disease status. Suspected infection in a disease-free country requires immediate official involvement rather than unregulated treatment that could conceal a carrier.
The protozoa Theileria equi and Babesia caballi invade red blood cells and are spread principally by competent ticks or contaminated blood equipment. Fever, anaemia, jaundice, dark urine and oedema range from mild to fatal. Surviving horses may remain carriers, creating movement and trade consequences in countries that are free of the organisms.
Travel origin, tick exposure, imported-horse documentation, blood transfusions and injection practices are central to risk assessment. Horses entering piroplasmosis-free countries or events are screened using tests approved by the relevant authority. Competitive ELISA and other serological methods detect antibody in carriers; PCR can demonstrate parasite nucleic acid and is useful in defined diagnostic or clearance programmes.
During acute fever, stained thick and thin blood films may reveal organisms within red cells, but parasitaemia can be low and microscopy is insensitive for carriers. Complete blood count, bilirubin and urinalysis document anaemia and haemolysis rather than identify the species. Mixed T. equi and B. caballi infections can occur.
A suspect or positive horse is isolated from tick and blood-transfer risks and reported where required. Exposed horses, shared equipment and premises ticks are traced. Negative microscopy does not release a horse with positive serology or significant epidemiological risk. Official laboratories and case definitions determine confirmation because false reassurance could compromise national freedom, trade and other equids.
Treatment and its legal availability depend on the parasite and the country’s control policy. Imidocarb dipropionate is used in supervised eradication or clinical programmes, with different regimens for T. equi and B. caballi. Adverse effects can include colic, diarrhoea, sweating and cholinergic signs, so administration requires veterinary monitoring and sometimes premedication. Supportive fluids, anti-inflammatory care and blood transfusion may be necessary for severe haemolytic anaemia.
Improved clinical signs do not prove parasite clearance. Treated horses undergo repeated official serological and molecular testing before quarantine can be lifted. In countries without an authorised clearance pathway, euthanasia, export or lifelong restriction may be required. Owners must not medicate a suspect before reporting because this can interfere with investigation and movement control.
Prevention uses tick inspection and appropriate acaricides, pasture and vegetation management, a new sterile needle and syringe for every horse, single-use intravenous sets and tested blood donors. Multidose vials must not be contaminated by used needles. During an outbreak, animal-health authorities direct tick treatment, contact testing and movement restrictions. There is no vaccine and no visual method for identifying an inapparent carrier.
