Equine asthma is a chronic, non-contagious inflammatory disease of the lower airways. Mild-to-moderate asthma is often recognised in younger performance horses with cough or reduced athletic capacity, while severe asthma more commonly causes recurrent airway obstruction in mature horses. Susceptible airways react excessively to inhaled particles such as stable dust, mould spores, endotoxin and, in some horses, seasonal pollens.
Inflammation increases mucus, narrows small airways and makes the bronchial muscle hyperresponsive. Mild disease may produce an occasional cough, tracheal mucus, delayed recovery or unexplained loss of performance without abnormal breathing at rest. Severe disease causes nostril flare, prolonged expiration, abdominal effort and sometimes a visible “heave line”. Affected horses are not usually febrile unless another infection or complication is present.
Exposure patterns vary. Dry hay, round bales, dusty bedding, sweeping, indoor arenas and poorly ventilated stables are common triggers, but pasture-associated disease occurs in some climates. Genetics and previous airway injury influence susceptibility. Asthma is therefore not evidence of inadequate care, yet continued high dust exposure makes medical control much less effective.
Repeated inflammation can remodel and thicken airways, reducing reversibility. Severe asthma is usually a lifelong condition managed rather than cured; some mild cases improve substantially when triggers are removed. The disease is not an infectious outbreak, so fever monitoring, quarantine and vaccination are not asthma treatment. Respiratory distress at rest is an emergency because exhausted horses can deteriorate and diagnostic procedures such as airway lavage may be unsafe until breathing is stabilised.
Mild to moderate disease may appear only as poor performance, occasional cough or delayed recovery, whereas severe asthma produces visible expiratory effort, abdominal heave and nostril flare at rest. Airway obstruction varies and can improve when exposure falls. The condition is inflammatory rather than contagious, and clear nasal discharge does not make it an infectious outbreak.
Owners should record cough frequency, nasal discharge, respiratory rate and effort at rest, recovery after work and links with hay, bedding, season or stable activities. Coughing during exercise is not a normal training habit. Fever, marked lethargy or illness spreading through a group suggests an infectious differential rather than uncomplicated asthma.
Veterinary assessment includes respiratory auscultation, rebreathing examination where safe and upper-airway endoscopy. Tracheal mucus supports lower-airway disease but is not specific. Bronchoalveolar lavage cytology is the preferred test for characterising inflammation in stable horses; it may be inappropriate during uncontrolled severe respiratory distress. Tracheal wash is more useful when bacterial infection is suspected. Pulmonary-function and bronchoprovocation testing are generally referral tools.
Routine blood tests and chest radiographs do not confirm asthma, although they can help exclude pneumonia, fibrosis or cardiac disease. Treatment response supports a diagnosis but should not replace investigation in a recurrent case. Follow-up uses resting effort, cough, performance and, when needed, repeat cytology or function testing. Owners should seek urgent care for breathing difficulty at rest, blue or dark gums, exhaustion or failure to eat because of respiratory effort.
Reducing respirable dust and allergens is the foundation of treatment. The specific plan follows the horse’s triggers: maximise suitable turnout, improve ventilation, remove the horse during sweeping, replace dusty bedding and avoid storing hay overhead. Forage may need effective steaming, wetting immediately before feeding, replacement with low-dust alternatives or removal of round-bale access. Feeding from the ground supports drainage but is unsuitable if the surface is dusty or sandy.
Corticosteroids control airway inflammation and may be given systemically or by an equine inhalation device. Bronchodilators provide faster relief of bronchospasm but should not be used alone because they do not control the underlying inflammation. Drug selection considers severity, administration reliability, competition rules and risks such as laminitis. Antibiotics are not indicated unless bacterial infection is documented or strongly suspected.
A horse in severe respiratory distress may require urgent bronchodilation, oxygen, systemic anti-inflammatory treatment and immediate environmental removal before extensive testing. Long-term control is judged by breathing effort, cough and function, with medication tapered only under veterinary direction. Return to work is gradual after airway control; forcing exercise through cough worsens stress. Prognosis is good for many mild cases with effective environmental change, while severe established airway remodelling may permanently limit performance.
