Eastern, Western and Venezuelan equine encephalomyelitis are mosquito-borne alphavirus diseases capable of causing inflammation of the horse’s brain and spinal cord. Eastern equine encephalitis is found principally in eastern North America and the Caribbean and is often rapidly fatal. Western equine encephalitis occurs in western parts of the Americas and is generally less lethal. Venezuelan equine encephalitis occurs in Central and South America and, unlike the other two, some outbreak strains produce enough virus in horses for mosquitoes to spread infection from horse to horse indirectly.
After fever and dullness, affected horses can develop loss of appetite, altered behaviour, apparent blindness, head pressing, circling, ataxia, weakness, inability to swallow, seizures, paralysis and recumbency. Early signs may resemble colic, liver disease, poisoning or trauma. Eastern and Venezuelan disease often progress quickly, and Eastern infection can kill within days.
Geography, mosquito activity and vaccination history materially change likelihood, but no clinical sign identifies the virus. A horse with neurological disease must also be assessed for West Nile, rabies, EHM and region-specific causes. These viruses can infect people through mosquitoes; routine horse contact is not the usual route, though blood and neural tissue are handled with appropriate precautions.
Outcome differs sharply by virus and severity. Eastern disease has a very poor prognosis, Western disease allows more survivors, and Venezuelan outbreaks require major animal-health control. Survivors can retain neurological deficits. Vaccination is highly effective where licensed products and local risk justify it, and mosquito reduction provides additional protection.
Eastern equine encephalitis usually causes the highest fatality, Western disease is often less severe, and Venezuelan strains can amplify in horses and generate major outbreaks. Mosquito ecology and geographic location differ among viruses. Rapid depression, fever, blindness, circling, seizures or recumbency requires public-health and regulatory consideration because clinical appearance cannot identify the virus.
Sudden neurological disease during mosquito season warrants immediate veterinary and animal-health notification, particularly after travel to an endemic region. History includes precise locations, dates, vaccination, mosquito exposure and whether other horses, birds or people are affected. The horse is kept quiet and protected from injury while zoonotic and reportable differentials are assessed.
Neurological examination localises dysfunction, and blood plus cerebrospinal fluid may be tested for virus-specific IgM or rising neutralising antibody. Virus isolation, PCR and post-mortem examination are used through authorised laboratories, but detectable viraemia may be brief. Vaccination can complicate interpretation of some serology, so laboratory and official case definitions matter.
Routine blood results do not confirm alphavirus infection. Rabies must remain in the safety assessment before oral or invasive procedures. A single negative early antibody result may require a later sample. In an area free of Venezuelan equine encephalitis, even one plausible case has serious movement and surveillance implications, so owners should not transport a suspect horse or arrange private testing without prior direction.
There is no specific antiviral cure for Eastern, Western or Venezuelan equine encephalomyelitis. Treatment uses anti-inflammatory medication, fluids, nutritional support, seizure control and careful nursing. Dysphagia requires withholding unsafe oral feed and protecting the airway. Weak horses need deep bedding, limb and head protection, bladder care and assisted repositioning; slings are considered only when the horse can use them safely.
Severe agitation, repeated seizures, respiratory dysfunction and prolonged inability to stand worsen prognosis. Euthanasia may be the most humane choice in rapidly progressive Eastern disease or when safe nursing cannot prevent suffering. Survivors require extended rehabilitation and gradual return to work because apparent alertness can improve before balance and strength.
Control follows the identified virus and jurisdiction. Eastern and Western cases prompt mosquito surveillance and protection, whereas Venezuelan outbreaks can require movement restrictions, emergency vaccination and vector control because horses may amplify epidemic strains. Preventive vaccination schedules are based on geography, season, travel and official advice. Standing water is reduced, water containers cleaned, approved repellents used and stabling timed to local mosquito activity. Antibiotics have no action against these viruses and are used only for a separate bacterial complication.
