Dystocia is a foaling that does not progress normally or requires assistance. Because the mare's explosive second stage usually delivers a foal within about 20 minutes after the water breaks, delay is an immediate emergency rather than a reason to watch for another hour. Every few minutes can affect foal oxygenation and the mare's survival.
The common problem is abnormal presentation, position or posture of the foal: a flexed limb or neck, dog-sitting posture, breech, transverse presentation or upside-down foal. A foal that is too large, twins, uterine torsion, pelvic abnormality or ineffective contractions can also obstruct delivery. In a normal anterior delivery, two front feet appear soles down, one slightly ahead, followed by the nose. A red velvety membrane at the vulva indicates premature placental separation, the “red bag” emergency, and the foal is losing placental oxygen.
Prolonged manipulation can cause vaginal, cervical or uterine tears, haemorrhage, nerve injury, retained placenta, metritis and laminitis. The foal may suffer hypoxia, rib fractures, trauma, neonatal encephalopathy or death. Pulling hard without identifying orientation can wedge the foal more tightly or tear the mare.
Risk is higher with previous dystocia, twins, abnormal placentation and some fetal deformities, but most cases cannot be predicted from appearance alone. Outcome is best when an experienced team, transport plan and referral facility are organised before foaling. The treatment priority may change from saving a live foal to saving the mare, and rapid decisions among assisted vaginal delivery, controlled delivery under anaesthesia, fetotomy and caesarean section are essential.
Premature placental separation is recognised when the red chorioallantois, rather than the translucent white amnion, appears first. Opening it and delivering the foal rapidly restores access to air, but the newborn still needs hypoxia assessment. In every dystocia, prolonged repeated attempts reduce the margin for both vaginal correction and successful caesarean delivery.
Attendants record when stage two began, when membranes ruptured, what structures appeared and whether the placenta separated early. If no rapid progress follows rupture, the veterinarian is called immediately while the mare is kept in a clean safe area. Untrained forceful traction is avoided.
The veterinarian assesses the mare's cardiovascular state and performs a lubricated vaginal examination to identify fetal presentation, position, posture, viability and available space. Transrectal examination or ultrasound can clarify uterine torsion and fetal heartbeat. The birth canal is checked for obstruction and the placenta for premature separation. At referral, blood work, cross-match preparation and surgical assessment proceed without delaying delivery.
After extraction, both patients require examination. The mare is checked for haemorrhage, cervical or vaginal tears, uterine rupture, retained membranes and rectal injury. The foal is assessed for breathing, heart rate, righting and suck reflexes, rib fractures, limb trauma, hypoxia and passive transfer. Placenta completeness and timing are documented.
The examination also determines whether enough room exists to repel and reposition the foal safely.
Initial correction uses abundant sterile lubricant, epidural or systemic sedation as appropriate and controlled repositioning while the mare is safely restrained. Traction is applied only to correctly identified limbs, in synchrony with contractions and along the pelvic axis. Excessive force and mechanical extraction without correction are avoided.
When standing manipulation cannot resolve the obstruction quickly, controlled vaginal delivery under general anaesthesia allows the mare to be positioned and the uterus relaxed. Caesarean section is selected promptly for an inaccessible or viable foal, uterine torsion, maternal compromise or failed correction; delay after repeated attempts worsens survival. Fetotomy may remove a dead fetus in selected cases when it can spare the mare and an experienced operator is available.
The newborn receives immediate airway, breathing and circulation support, then oxygen, fluids and neonatal intensive care according to hypoxia and trauma. The mare receives analgesia, fluids and treatment for haemorrhage or tears. Retained-placenta prevention and monitoring begin immediately, especially after surgery or prolonged manipulation; antimicrobials, uterine lavage and laminitis prevention are used when contamination or injury warrants them. Reproductive re-examination after healing informs future breeding, and the foaling record guides a safer plan for subsequent pregnancies.
