A concise guide to the condition’s pattern, detection, management and urgency.
This snapshot is a general guide, not a diagnosis or treatment plan. New, severe or worsening signs require veterinary assessment.
A horse’s cheek teeth erupt gradually throughout life to replace surfaces worn during chewing. Because the upper jaw is wider than the lower, normal sideways movement can leave sharp enamel points on the cheek edge of upper teeth and tongue edge of lower teeth. Mild points are common, but pronounced points, hooks, ramps, wave mouth, step mouth and missing or displaced teeth can restrict jaw movement, injure soft tissue and prevent efficient grinding of forage.
Signs include dropping partially chewed feed, packing feed in the cheeks, slow eating, long fibres in manure, weight loss, bad breath, drooling and recurrent choke. Some horses resist the bit, tilt the head or behave differently during ridden work because sharp edges cut the cheek or tongue. Unilateral nasal discharge or facial swelling can indicate infected cheek-tooth roots and secondary sinus disease. Many horses continue eating despite substantial abnormalities.
Young horses can have painful retained caps and erupting permanent teeth, while older horses are prone to worn, loose or fractured teeth and gaps that trap feed. Abnormal jaw alignment, previous extraction and focal dental pain change the wear pattern. Dental disease may contribute to poor condition and digestive difficulty, but it should not be assumed to explain weight loss until systemic, nutritional and gastrointestinal causes are also considered.
“Floating” is not a uniform cosmetic procedure. The objective is comfortable, functional chewing while preserving the ridged occlusal surface and avoiding heat or pulp injury. Severe longstanding wear cannot always be corrected fully in one session, and aggressive reduction can cause permanent damage.
Because equine cheek teeth erupt continuously for much of life, uneven wear can create sharp buccal points on upper teeth and lingual points on lower teeth. Hooks, waves, steps and fractured teeth alter chewing differently from simple points. Dropping partially chewed feed, long fibres in manure or quidding reflects impaired processing and can lead to weight loss or choke.
Every horse should receive regular oral examination, with frequency based on age, diet, previous findings and signs rather than a fixed rasping schedule. Developing dentition and elderly mouths often need closer review. Owners should monitor chewing time, quidding, appetite, body condition, odour, nasal discharge and response to the bit. Examining only the incisors does not assess the cheek teeth.
A complete veterinary dental examination usually uses sedation, a full-mouth speculum, bright light, mirror or oral endoscope and systematic palpation of each arcade. The examiner records sharp points, overgrowths, diastemata, fractures, mobility, feed packing and soft-tissue wounds. Dental radiographs are indicated when root disease, fracture, abnormal eruption or sinus involvement is suspected. CT may assist complex cases.
Findings should be charted so progression and previous treatment can be compared. A sharp point seen without associated injury is not automatically the cause of every performance problem. Prompt examination is warranted for inability to chew, marked swelling, foul unilateral nasal discharge, bleeding, suspected fracture or recurrent choke.
Dental equilibration removes clinically important sharp points and excessive overgrowth while maintaining functional contact between opposing teeth. Sedation, restraint, local analgesia and powered or hand instruments are selected for safe access. Correction may be staged when an abnormality is severe, because removing excessive tooth in one visit risks pulp exposure, thermal injury and loss of chewing function.
Retained caps, loose fragments and diseased teeth are extracted when indicated. Some infected teeth can receive endodontic treatment, while advanced decay, fracture or apical infection usually requires removal by an oral or surgical approach. Missing teeth leave unopposed teeth that continue erupting, so ongoing adjustment is needed. Sinus infection and oral wounds are treated as separate consequences rather than expected to resolve through floating alone.
Diet is adapted when chewing surfaces cannot be restored: soaked complete feeds, chopped fibre or other nutritionally balanced alternatives can maintain older horses. After extraction, owners follow instructions on rinsing, feeding, pain relief and monitoring for swelling, odour or discharge. Routine follow-up is individual; repeated automatic reduction without examination is inappropriate. Prognosis is good for corrected enamel points, but advanced periodontal, root or wear disease often requires lifelong dental and nutritional management.
