Colic is a general term for abdominal pain rather than a single disease. In horses it can arise from gas, intestinal spasm, impaction, displacement, strangulation, inflammation, ulceration or disease outside the digestive tract. The cause may be mild and self-limiting, but some forms interrupt blood supply to the intestine and become fatal without prompt surgery. This wide range is why the intensity of pain alone cannot identify the underlying problem.
Common signs include pawing, looking at the flank, repeated lying down, rolling, sweating, stretching as if to urinate, reduced appetite and passing less manure. A quieter horse may simply appear dull, stand abnormally or stop finishing feed. Severe or persistent pain, abdominal distension, rapid heart rate, abnormal gum colour and deterioration despite pain relief are particularly concerning. Horses cannot vomit, so fluid accumulating in the stomach can create dangerous pressure and may require urgent decompression.
Risk is influenced by changes in forage or concentrate, inadequate water intake, dental disease, parasite exposure, ingestion of sand, reduced movement, previous abdominal surgery and individual anatomy. However, many episodes occur despite careful management, and an owner should not delay veterinary care while trying to identify a management mistake.
The outcome depends on the specific lesion, the amount of intestinal and circulatory damage, and how quickly effective treatment begins. A simple large-colon impaction generally has a much better prognosis than a strangulating obstruction. Colic can also lead to dehydration, endotoxaemia, shock and secondary laminitis. Because horses often suppress outward signs between painful episodes, temporary quietness should not automatically be mistaken for resolution. For that reason, any episode should be treated as a clinical sign requiring assessment and monitoring, not as a diagnosis that automatically receives the same home remedy.
There is no routine laboratory screening test that predicts every form of colic. Early detection relies on knowing the horse’s normal appetite, manure production, drinking, behaviour and resting vital signs. Owners should record when pain began, whether it is constant or intermittent, the last normal manure, recent feed or water changes, medications, parasite control, dental history, transport and previous episodes. The response to any treatment already given must be reported accurately.
Veterinary assessment commonly includes heart and respiratory rates, temperature, gum colour, capillary refill, hydration, abdominal sounds and rectal examination. Nasogastric intubation can detect or relieve stomach reflux. Ultrasound, blood tests, abdominal-fluid analysis and repeated examinations help assess intestinal position, inflammation and circulation. No single finding is decisive in every horse; trends in pain, heart rate, reflux and cardiovascular status are often more informative than one examination.
Immediate veterinary review is warranted for repeated rolling, persistent pain, marked distension, absent manure, weakness or pain returning soon after medication. Early referral is important when findings suggest obstruction, strangulation or the possible need for surgery.
Treatment is determined by the cause and the horse’s cardiovascular condition. The first priorities may include safe pain relief, sedation, gastric decompression and restoration of circulating fluid volume. A veterinarian may administer intravenous or enteral fluids, electrolytes, lubricants or other medication after assessing the likely lesion. Feed is commonly withheld during active investigation, while access to water and controlled movement are decided for the individual case.
Medical treatment is appropriate for many gas, spasm and uncomplicated impaction cases. Repeated examinations are essential because a horse that initially appears stable can deteriorate or fail to respond. Persistent pain, substantial gastric reflux, worsening gum colour, rising heart rate, abnormal abdominal fluid or evidence of displaced or devitalised intestine are reasons for rapid surgical referral. Surgery may reposition intestine, release a strangulation or remove irreversibly damaged bowel.
Recovery care depends on the lesion and treatment. Feed is reintroduced gradually, hydration and manure are monitored, and exercise resumes according to veterinary advice. Horses recovering from endotoxaemia require surveillance for laminitis. After the episode, confirmed contributing factors such as dental disease, sand exposure, abrupt ration changes or inadequate water should be addressed, but unproven supplements should not replace diagnosis-based prevention. Prognosis ranges from excellent for promptly resolved uncomplicated disease to guarded for advanced strangulation or shock.
