Choke in horses means obstruction of the oesophagus, not blockage of the windpipe. The horse can usually breathe, but cannot swallow saliva, water or feed normally. Material pools above the obstruction and may flow from the nostrils or enter the airway. Common obstructing material includes hurriedly swallowed grain, dry beet pulp, pellets, hay and unsuitable treats. Poor teeth, dehydration, recent sedation, abnormal swallowing and pre-existing oesophageal narrowing increase risk.
Typical signs are sudden anxiety, repeated swallowing, neck stretching, coughing, drooling and frothy nasal discharge containing feed. Some horses continue attempting to eat or drink, worsening the accumulation. A firm obstruction may occasionally be felt on the left side of the neck, but many are located deeper in the chest. The distress can resemble colic.
The most immediate complication is aspiration of saliva and feed into the lungs, which can cause pneumonia or pleuropneumonia. Longer obstruction increases pressure damage to the oesophageal lining and can lead to ulceration, tissue death, scar-related stricture or rarely rupture. Respiratory complications may not become obvious until after the blockage has cleared.
Many simple feed obstructions resolve after sedation and relaxation, but owners cannot safely determine which will do so. Feed and water should be removed and veterinary assistance requested. Nothing should be poured into the mouth because the horse cannot swallow safely; mineral oil is particularly dangerous if aspirated. Although the word “choke” suggests suffocation, sudden respiratory distress is not expected from an uncomplicated oesophageal blockage and may indicate severe aspiration or another emergency. Recurrent choke is not normal greedy eating and warrants investigation for dental disease, stricture, diverticulum, motility disorder or an external mass compressing the oesophagus.
Unlike small-animal choking, the horse usually continues to breathe because the blockage is in the oesophagus rather than the airway. Saliva and feed may enter the trachea while the horse coughs or repeatedly tries to swallow, making aspiration pneumonia the most important delayed complication. Strictures or repeat obstruction can follow mucosal injury, so a resolved episode still needs follow-up.
Early recognition is usually based on the characteristic combination of attempted swallowing, coughing and feed-stained fluid from both nostrils. Feed and water should be removed immediately while the veterinarian is contacted. Owners should record what the horse was eating, how long signs have been present, previous episodes, dental history and any recent sedation. Breathing difficulty, fever or worsening depression raises concern for aspiration.
The veterinarian may identify the obstruction by gently passing a nasogastric tube, palpating the neck or performing endoscopy. Endoscopy shows the site and nature of the blockage and, after clearance, reveals mucosal injury or an underlying narrowing. Radiographs, contrast studies or ultrasound may be needed for recurrent obstruction, foreign material, suspected rupture or disease outside the oesophagus. Lung examination and thoracic imaging help assess aspiration.
After resolution, temperature, cough, breathing effort, appetite and nasal discharge should be monitored for several days because pneumonia can appear later. Repeat endoscopy may be recommended after significant mucosal damage. A stricture can become most apparent during healing, so recurrence in the following weeks requires prompt reassessment rather than simply soaking the same feed.
The horse is kept off feed and water and usually sedated so the head remains low, reducing anxiety and helping oesophageal muscle relaxation while allowing material to drain away from the airway. Some recent, simple obstructions resolve with sedation and time. Intravenous fluids may be required for dehydration. Medication may be used to reduce spasm, pain and inflammation according to the horse’s condition.
If the obstruction persists, the veterinarian passes a tube and performs careful lavage using repeated small volumes of water that are allowed to return. Forceful flushing can rupture the oesophagus or drive material into the lungs. General anaesthesia, protected airway techniques, endoscopic instruments or, rarely, surgery may be required for difficult foreign material. Mineral oil must not be administered while the oesophagus is blocked because aspiration can cause severe lipid pneumonia.
Once cleared, endoscopy assesses injury and confirms that no material remains. Feed is reintroduced in a soft, wet form according to the extent of damage. Antimicrobials are reserved for significant aspiration or other indicated infection risk, not automatically given to every uncomplicated case. Dental correction and slower feeding methods address preventable causes. Horses with aspiration require respiratory monitoring and possibly intensive treatment. Prognosis is good after prompt uncomplicated clearance, but pneumonia, stricture or rupture substantially worsens the outlook.
