Bone spavin is osteoarthritis of the low-motion distal intertarsal and tarsometatarsal joints of the hock, sometimes also involving the proximal intertarsal joint. Cartilage loss, subchondral bone change and periarticular new bone make movement painful until, in some cases, the affected low-motion joints eventually fuse.
Horses may show gradual hindlimb lameness, stiffness when first worked, reduced hock flexion, toe dragging, shortened cranial phase and difficulty with collection, jumping or hills. Lameness can improve as the horse warms up. Firm swelling on the lower inside hock is the classic “spavin”, but many painful joints have little visible enlargement. Both hocks are commonly affected, producing loss of performance without an obvious head nod.
Conformation, repetitive compression and rotation, intense work, previous developmental joint disease and trauma contribute. Bone spavin differs from bog spavin, which is fluid distension of the upper tarsocrural joint, and from soft-tissue causes of proximal metatarsal pain. A positive hock flexion test is not specific because it stresses several joints and structures.
The distal joints contribute little to overall hock range, so complete fusion can remove pain with limited mechanical loss. Before fusion, progression is variable and symptoms can recur. Prognosis for useful work is often fair to good if pain is accurately localised and upper hock joints are healthy. Proximal intertarsal involvement or severe concurrent suspensory disease worsens expectations.
Bone spavin is osteoarthritis of the lower hock joints, principally the distal intertarsal and tarsometatarsal joints. These joints have little motion, so pain may lessen after they fuse completely. Horses often show shortened cranial phase, toe dragging and reduced hock flexion, but a positive flexion test is not specific and upper hock disease behaves differently.
Loss of hock action can also overload the back and opposite limbs over time.
Examination compares hock shape, distal-joint thickening, hoof wear and hindquarter muscle. Lameness is observed on straight lines and circles, and flexion responses are interpreted with other findings. Intra-articular analgesia of the distal hock joints localises pain, although diffusion between adjacent joints and the proximal suspensory region requires careful timing and technique.
Radiographs confirm joint-space narrowing, subchondral sclerosis or lysis and periarticular new bone. Early painful disease can have subtle images, while marked bone formation can exist after a joint is already fused and comfortable. Nuclear scintigraphy, CT or MRI can help when radiographs and blocks conflict or concurrent proximal suspensory disease is suspected.
Hoof balance and distal tarsal conformation are assessed because uneven loading affects management. Serial imaging is useful when facilitated fusion is planned, but the horse’s comfort remains central. A sudden severe hock swelling or fever is not typical bone spavin and needs urgent evaluation for fracture, synovial infection or upper-joint disease.
Local anaesthetic of the distal hock joints helps connect radiographic change with the horse's pain.
Conservative treatment combines workload adjustment, regular controlled exercise, balanced farriery and pain control. NSAIDs are used at the lowest effective veterinary-directed exposure. Distal hock joints can be injected with corticosteroid, often providing temporary relief; accurate sterile placement and competition withholding rules matter. Shockwave and systemic bisphosphonates are options in selected cases, with evidence and licensing considered.
Shoeing changes aim to ease breakover and reduce twisting rather than forcing one standard configuration. Maintaining hindquarter strength helps the horse use the limb efficiently. When pain persists, fusion of the affected low-motion joints may be encouraged chemically, surgically or with drilling or laser techniques. Fusion must be confined to the correct distal joints because destroying a high-motion joint would be catastrophic.
After facilitated ankylosis, exercise and radiographic follow-up continue until fusion is established. Pain can temporarily increase during the process. Upper hock arthritis and proximal suspensory injury are treated separately. Many horses return to useful athletic work once the painful distal joints fuse, although very high-level performance and cases involving the proximal intertarsal joint have a more guarded prognosis.
Corrective shoeing improves breakover and mediolateral balance without attempting to force abnormal hock motion.
