Vestibular disease affects the system responsible for balance and spatial orientation. The vestibular apparatus includes structures within the inner ear and pathways in the brainstem and cerebellum. Disease can therefore be peripheral, affecting the ear and vestibular nerve, or central, affecting the brain.
Typical signs include a sudden head tilt, loss of balance, falling or rolling, circling and rapid involuntary eye movements called nystagmus. Cats can become nauseated and may vomit or refuse food because the sensation resembles severe motion sickness. The dramatic appearance can be frightening, but some peripheral cases improve rapidly.
Causes include middle or inner ear infection, nasopharyngeal polyps, trauma, toxins and inflammatory or neoplastic brain disease. Cats can also develop idiopathic vestibular syndrome, where no specific cause is found and recovery occurs over days to weeks. Central neurological signs such as altered awareness, weakness or abnormal postural reactions increase concern for brain disease.
A head tilt alone cannot determine the cause.
Vestibular disease is a syndrome, not one diagnosis. Prognosis is excellent for many idiopathic peripheral cases but much more variable for infection, tumour or central disease.
The direction of the head tilt or nystagmus can help neurological localisation, but it does not by itself reveal the cause. A cat with an ear polyp and a cat with idiopathic vestibular syndrome can initially look very similar. The trend over time and accompanying neurological signs become important diagnostic clues.
Video of the first episode is useful because eye movements and gait may change by the time the cat reaches the clinic. Peripheral vestibular disease usually leaves strength and awareness normal, while central disease can produce weakness, altered mentation or other cranial-nerve deficits. Idiopathic vestibular disease usually shows a clear recovery trend despite a dramatic initial presentation.
Sudden loss of balance, head tilt, rolling or nystagmus warrants veterinary assessment even when the cat remains alert.
Neurological examination helps localise the problem. Otoscopic examination looks for external or middle-ear clues.
When ear disease is suspected, CT or MRI can evaluate the middle and inner ear more accurately than a simple external examination. MRI is particularly useful for suspected central disease. Blood tests are selected according to age and possible systemic causes.
There is no screening test for idiopathic vestibular disease before signs occur. Cats with chronic ear disease should have persistent head shaking, pain or discharge investigated before infection extends deeper. Inability to stand, repeated vomiting, altered consciousness or rapidly worsening neurological signs increases the urgency of assessment.
Diagnosis begins with neurological and ear examination. Otoscopy, imaging of the middle ear, blood tests and advanced imaging such as MRI may be recommended depending on whether peripheral or central disease is suspected. Early assessment is important because treatable ear disease can look very similar to a self-limiting idiopathic episode.
Treatment depends on the cause. Idiopathic peripheral vestibular disease is generally managed supportively with anti-nausea medication, hydration and a safe environment while balance returns. Many cats improve substantially within a few days, although a mild head tilt can persist.
Middle or inner ear infection requires appropriate antimicrobial treatment and sometimes surgery or removal of a polyp. Central inflammatory disease, cancer or toxin exposure needs cause-specific management. Medication is not chosen simply from the presence of a head tilt.
At home, affected cats should be confined temporarily to a small padded area without stairs or high furniture. Food, water and litter should be easy to reach. Assistance may be needed for eating during the first days if nausea is severe.
Lack of improvement, progression or development of new neurological deficits warrants re-evaluation. The prognosis is good for uncomplicated peripheral disease but depends on cause for central disease. Residual head tilt does not necessarily indicate ongoing discomfort if the cat has otherwise regained normal balance and function.
If ear disease is the cause, hearing may remain reduced even after balance improves, depending on the amount of middle- or inner-ear damage.
