Urethral obstruction occurs when a cat cannot pass urine because the urethra is blocked. It is most common in male cats because their urethra is long and narrow. The obstruction may involve inflammatory material, a urethral plug, small stones, swelling or a combination of these factors. It is a true medical emergency.
Early signs can resemble uncomplicated cystitis: frequent litter-tray visits, straining, genital licking, vocalising and passing only tiny amounts of urine. As the blockage becomes complete, the bladder becomes increasingly large and painful. Waste products and potassium accumulate in the blood, kidney function deteriorates and the cat may vomit, become profoundly weak, collapse or develop dangerous heart rhythm abnormalities.
A common and potentially fatal mistake is assuming that the cat is constipated. A male cat that repeatedly strains in the tray and produces little or no urine must be assessed urgently. Waiting to see whether the problem resolves can allow life-threatening metabolic changes to develop within a relatively short period.
Relieving the obstruction treats the immediate crisis, but it does not automatically explain why it happened. Many cats have underlying idiopathic cystitis, while others have stones or recurrent urethral plugs. Long-term prevention must therefore be tailored to the cause and the cat’s individual risk factors.
Even after the bladder has been emptied, the cat is not automatically out of danger. Potassium, kidney values and urine production may take time to normalise, and some cats develop post-obstructive diuresis, producing very large amounts of urine that require careful fluid replacement and monitoring.
The most important form of early detection is recognising unsuccessful urination. Crying, restlessness, hiding, vomiting, genital licking and a painful abdomen strengthen the concern. Cats that have obstructed previously need close observation for recurrence. Daily awareness of litter-tray output is valuable, especially after discharge.
A cat that visits the litter tray repeatedly, strains and produces no urine or only a few drops needs emergency veterinary assessment, particularly if male.
Owners should not try to squeeze the bladder or attempt to relieve the blockage at home. A blocked bladder can rupture, and manipulation can cause severe pain or injury. The problem also cannot be safely distinguished from constipation without examination.
At the clinic, the veterinarian assesses bladder size, hydration and cardiovascular stability. Blood tests measure kidney values and potassium because severe hyperkalaemia can affect the heart. An electrocardiogram may be used when electrolyte abnormalities are significant. Urinalysis and imaging are often performed once the immediate crisis is controlled to identify stones, crystals or other causes.
There is no screening blood test that predicts the next blockage; early detection depends on noticing reduced urine production quickly and treating every episode of repeated unsuccessful straining as urgent.
Diagnosis is based on the history, examination and finding a distended bladder with inability to pass urine. Blood tests evaluate kidney function, potassium and acid-base changes. Urinalysis and imaging may be used after stabilisation to look for crystals, stones or other contributing disease.
Emergency treatment begins with stabilising the cat and restoring urine flow. Pain relief is provided immediately. Cats with severe dehydration, kidney changes or high potassium usually receive intravenous fluids and treatment for dangerous electrolyte abnormalities. A urinary catheter is passed under sedation or anaesthesia to relieve the obstruction and flush the bladder.
The catheter may remain in place for a period while swelling settles and urine production is monitored. After removal, the cat is observed to make sure spontaneous urination resumes. Treatment then shifts to the underlying cause. This may include a therapeutic urinary diet, increased water intake, management of idiopathic cystitis and environmental changes that reduce stress.
Antibiotics are not automatically required and are used when bacterial infection is demonstrated. Cats with repeated obstruction despite appropriate management may be considered for perineal urethrostomy, a surgery that creates a wider urinary opening. The procedure reduces the risk of future urethral blockage but does not prevent bladder inflammation or stone formation.
At home, prescribed diet and medication should be followed exactly, water intake encouraged and litter trays kept clean and accessible. Recurrence is possible even after successful treatment. Any renewed straining without normal urine production is another emergency and should never be managed by waiting for a scheduled appointment.
