Toxoplasmosis is disease caused by the protozoan parasite Toxoplasma gondii. Cats are the definitive host, meaning the parasite can complete its sexual life cycle in their intestine and produce oocysts that are shed in faeces. Most infected cats never become clinically ill and usually shed oocysts only for a limited period after their first infection.
Cats acquire Toxoplasma mainly by eating infected prey or raw meat containing tissue cysts. Clinical toxoplasmosis develops when the parasite multiplies outside the intestine and damages organs. Young, immunocompromised or severely stressed cats are more likely to develop disease.
Signs depend on which tissues are affected. Fever, lethargy and poor appetite are common nonspecific findings. Pneumonia can cause breathing difficulty, liver involvement can cause jaundice, and inflammation of the eyes may produce uveitis. Neurological disease can cause incoordination, seizures, behavioural change or weakness. Because the presentation is so varied, toxoplasmosis can mimic many other conditions.
Human health concerns are often misunderstood. People are more commonly exposed through undercooked meat, soil or unwashed produce than by touching a healthy cat. Daily litter removal reduces risk because freshly passed oocysts need time in the environment before becoming infectious.
Cats are central to the parasite’s life cycle but are not continuously shedding infectious material. Most shedding follows a primary intestinal infection and lasts for a limited period. This is why sensible litter hygiene and food precautions are more useful than treating every cat in a household as a constant zoonotic hazard.
Prevention includes feeding a complete cooked commercial diet rather than raw meat, limiting hunting and removing litter faeces daily. Pregnant or immunocompromised people can reduce exposure further by using gloves for litter and soil and practising good food hygiene.
Toxoplasmosis is investigated when compatible systemic, ocular, respiratory or neurological signs occur, especially in a cat that hunts, eats raw meat or is immunocompromised. Routine testing of every healthy pet cat is not generally useful because a positive antibody result often reflects previous exposure rather than active disease.
Serology can measure IgG and IgM antibodies. Results are interpreted with clinical signs and sometimes repeated after an interval to look for a changing titre. PCR testing may be performed on blood, cerebrospinal fluid, ocular fluid or other samples depending on the affected organ, but a negative result does not always exclude disease.
Eye examination is important when uveitis is present, while chest imaging is used for respiratory signs and neurological investigation is tailored to the presentation. Other infections and inflammatory diseases must be excluded because no single sign is specific to toxoplasmosis.
Testing a healthy cat does not reliably predict whether it will ever shed oocysts in future.
Diagnosis is rarely made from one antibody result. Serology shows immune exposure and can help when paired samples or particular antibody patterns support recent infection, but many healthy cats have antibodies from previous exposure. PCR or cytology from affected tissues and response to treatment may add evidence.
Clinical toxoplasmosis is commonly treated with clindamycin, with dose and duration determined by the veterinarian. Improvement may be seen within several days when the diagnosis is correct, but neurological or ocular lesions can take longer to resolve and may leave residual damage.
Supportive treatment is based on the affected organs. Cats with pneumonia may need oxygen and hospital care, while those with uveitis require specific eye treatment to control damaging inflammation. Seizures, dehydration, poor appetite and other complications are treated as needed. Immunosuppressive drugs are used cautiously because uncontrolled parasite replication is more likely when immune defence is reduced.
Cats should not be fed raw meat during or after recovery because it can expose them to new tissue cysts. Hunting should be limited where practical. Daily litter cleaning reduces environmental contamination, although most clinically affected cats are not necessarily shedding large numbers of oocysts at the time signs develop.
The prognosis is often good when treatment begins before severe neurological or respiratory damage occurs. Cats with profound immunosuppression or extensive organ involvement have a more guarded outlook. Recurrence is uncommon in immunocompetent cats after successful treatment, but underlying immune problems should be addressed when present.
