Systemic hypertension means persistently high arterial blood pressure. It is most common in older cats and frequently occurs secondary to chronic kidney disease, hyperthyroidism or hyperaldosteronism. Some cats have high blood pressure without a clearly identified underlying cause.
Hypertension is dangerous because a cat can appear relatively normal while high pressure damages delicate blood vessels in the eyes, kidneys, brain and heart. The retina is particularly vulnerable. Bleeding or detachment can cause sudden partial or complete blindness, dilated pupils or disorientation. Neurological effects can include seizures, altered behaviour, weakness or loss of balance. Kidney protein loss may worsen, and the heart can develop thickening in response to sustained pressure.
Blood pressure must be measured carefully because stress can temporarily increase readings in a veterinary clinic. A calm environment, appropriate cuff size and repeated measurements help distinguish persistent hypertension from a single stress response. Results are interpreted together with evidence of target-organ damage and the cat’s medical history.
Potassium and adrenal evaluation may be indicated when hyperaldosteronism is suspected.
Hypertension is treatable, but prolonged severe pressure can cause irreversible injury. Rapid recognition is especially important when vision has changed. Some retinal detachments can partially improve if blood pressure is controlled quickly, while longstanding damage may be permanent.
Detecting pressure before retinal detachment is very different from diagnosing it after a cat has suddenly lost vision, even though the underlying disease may be the same.
A baseline in a calm, healthy cat can also make future changes easier to interpret. Several readings are usually obtained and obviously inconsistent values are discarded. Retinal haemorrhage, tortuous vessels or detachment indicate target-organ injury and increase the urgency of treatment. Hypertension itself usually cannot be recognised by touch or behaviour before organ damage appears.
Blood pressure screening becomes increasingly useful as cats age and is particularly important in cats with chronic kidney disease, hyperthyroidism, adrenal disease, unexplained eye changes or neurological signs.
Measurements should be taken using a consistent technique after the cat has had time to settle. A single elevated measurement in a frightened cat is not automatically a diagnosis.
Eye examination is recommended when hypertension is suspected. Blood chemistry, thyroid testing and urinalysis help identify common underlying causes, while additional endocrine testing is used selectively.
At home, sudden blindness, persistently dilated pupils, bumping into objects, seizures, weakness or major behavioural change warrants urgent assessment. Regular measurement in at-risk cats is therefore more useful than waiting for obvious symptoms.
Eye examination is an important part of assessment because retinal haemorrhage, swelling or detachment may confirm that high pressure is clinically significant. Blood and urine testing are used to look for kidney disease, thyroid disease and other underlying disorders. Because affected cats may feel normal until an eye or brain complication occurs, hypertension is a condition where routine measurement can genuinely prevent irreversible damage.
Amlodipine is the most commonly used first-line medication for feline hypertension because it reliably lowers arterial pressure in many cats. The dose is adjusted according to repeat measurements rather than estimated from clinical appearance. Other drugs may be added or substituted in selected cases.
The underlying disease also needs treatment. Hyperthyroidism, chronic kidney disease or hyperaldosteronism can continue to drive high blood pressure if left unmanaged. Kidney function, potassium and urine protein are monitored because hypertension and renal disease influence one another.
Blood pressure is usually rechecked relatively soon after medication begins or changes, then at longer intervals once stable. Excessive lowering can also be harmful, so controlled adjustment is important. Owners should give medication consistently and not stop it simply because the cat seems normal.
If retinal detachment or haemorrhage has occurred, rapid blood-pressure control offers the best chance of preserving or recovering some vision, although permanent blindness can remain. Neurological or cardiac complications may also improve when pressure is controlled. Most cats require long-term treatment and monitoring because hypertension often reflects a chronic underlying disorder rather than a temporary episode.
