Retinal detachment occurs when the sensory retina separates from the tissues beneath it that normally provide essential metabolic support. Once detached, retinal function falls rapidly. In cats, systemic hypertension is one of the most important causes, particularly in older cats with chronic kidney disease, hyperthyroidism or hyperaldosteronism.
A cat may become suddenly blind, develop very large pupils or bump into objects after previously seeing normally. Retinal haemorrhage can accompany the detachment. Other causes include inflammation within the eye, trauma, congenital abnormalities and, less commonly, tumours.
Because the retina itself cannot be seen by an owner, the first clue is often a behavioural change. Cats may become hesitant, vocal or disoriented and may stop jumping onto familiar surfaces. Some adapt quickly enough that partial blindness is initially missed.
Detached retina may be visible billowing within the eye. Blood pressure should be measured immediately because severe hypertension requires urgent treatment.
The possibility of visual recovery depends on cause, duration and extent. Hypertensive detachments treated very quickly can sometimes reattach with partial return of vision. Prolonged separation damages photoreceptors irreversibly. The condition is therefore both an eye emergency and, in many cats, a sign of important systemic disease that needs simultaneous treatment.
A detached retina can remain partly functional for a short period, so visual behaviour may fluctuate rather than disappear completely at once. Persistent vascular pressure, haemorrhage or inflammation continues separating sensitive tissue even while the cat begins adapting to reduced sight. The recovery window remains limited because deprived photoreceptors deteriorate with time, and apparent coping does not mean the underlying eye or systemic disease has stabilised.
Waiting several days can reduce the chance of visual recovery if high blood pressure is the cause. Prevention depends on controlling systemic disease before pressure becomes high enough to damage the retina.
Sudden blindness, persistently dilated pupils or a cat unexpectedly colliding with furniture should prompt urgent veterinary examination.
A fundic examination after pupil dilation is the main diagnostic test. The veterinarian looks for detached retina, haemorrhage, vascular changes and inflammation. Blood pressure is measured because hypertension is common and may be clinically silent until eye damage occurs.
Blood chemistry, urinalysis and total T4 are frequently appropriate in older cats to investigate kidney disease and hyperthyroidism. Potassium and adrenal testing may be added when hyperaldosteronism is suspected. Ocular ultrasound helps when the back of the eye cannot be seen directly.
Cats with known hypertension or chronic kidney disease should have regular blood-pressure monitoring and eye assessment when indicated. There is no separate screening test for retinal detachment itself.
When one eye is affected, the opposite eye should also be examined carefully because systemic hypertension and inflammatory disease can threaten both eyes even if only one has detached visibly.
Diagnosis requires a dilated fundic examination. Blood tests, urinalysis and thyroid testing help identify underlying disease. Ocular ultrasound is useful when cataract, haemorrhage or other opacity prevents a clear view of the retina.
Treatment targets the cause. Hypertensive retinal detachment requires rapid blood-pressure control, most commonly with amlodipine, alongside management of kidney, thyroid or adrenal disease. The retina may gradually return towards its normal position if treatment starts early enough.
Inflammatory detachment is treated according to the underlying uveitis, while trauma or other structural causes require individual ophthalmic management. Surgery is rarely the routine approach for the common hypertensive feline detachment but may be considered in specialised circumstances.
The cat should be kept safely indoors while vision is impaired. Food, water and litter trays remain in predictable locations, and stairs or balconies are blocked if there is a fall risk. Sudden handling should be preceded by voice to reduce startle.
Even when vision does not return, the eye may remain comfortable and the cat can adapt well. Long-term management focuses on preventing further retinal injury in either eye through consistent blood-pressure and systemic-disease control. Repeat eye examinations help document reattachment, residual damage and any ongoing haemorrhage.
The underlying hypertension or inflammatory disease usually requires lifelong monitoring even if the retina reattaches and vision improves.
