Restrictive cardiomyopathy is a form of heart muscle disease in which the ventricles become abnormally stiff and cannot relax normally between beats. In many cats, bands of fibrous or scar-like tissue develop within the heart, limiting normal filling even though the ventricular walls may not be markedly thickened. The atria often enlarge because blood returning to the heart meets resistance.
The disease can remain hidden for a period and may first be detected when a murmur, gallop rhythm or abnormal heart rate is heard. Some affected cats show reduced activity or faster breathing at rest. Others present suddenly with congestive heart failure, fluid around the lungs or an arterial blood clot. Enlargement of the left atrium increases the risk of clot formation, and a clot can cause sudden painful weakness or paralysis of the hind limbs.
The underlying cause is often not identified. The disease is generally progressive, although the rate varies considerably.
The most clinically important complications are congestive heart failure and thromboembolism. Once either develops, long-term prognosis becomes more guarded, but treatment can still improve comfort and sometimes provide meaningful periods of good quality of life.
A useful distinction from HCM is that the main problem is impaired filling from stiffness or fibrosis rather than simple thickening of the ventricular wall.
Routine heart auscultation is useful but cannot rule out restrictive cardiomyopathy because some cats have no obvious murmur. It is particularly important when a cat has an abnormal heart sound, increased sleeping respiratory rate, unexplained pleural effusion or a previous arterial thromboembolism. The scan evaluates atrial size, ventricular filling, wall motion and fibrous changes that help distinguish restrictive disease from other cardiomyopathies. A persistent rise in resting breathing rate, increased effort, open-mouth breathing, collapse and sudden hindlimb weakness can mark decompensation or arterial thromboembolism.
A gallop rhythm, persistent tachycardia, unexplained respiratory change or evidence of atrial enlargement should prompt further cardiac assessment.
Echocardiography is the main diagnostic test.
Chest radiographs are useful when congestive heart failure is suspected, while blood pressure and thyroid testing help exclude conditions that can alter cardiac structure or function. Cardiac biomarkers may support screening in some settings but are not definitive.
At home, owners of known cardiac patients can record sleeping respiratory rate when the cat is calm. There is no established DNA screening programme for this condition; early detection depends on examination and echocardiography when risk or signs justify it.
Restrictive cardiomyopathy is usually diagnosed by echocardiography. The ultrasound allows the veterinarian or cardiologist to assess chamber size, movement of the heart walls, fibrous changes, blood-flow patterns and signs of elevated filling pressure. Chest radiographs, blood pressure, electrocardiography and cardiac biomarkers may provide additional information, but they do not replace a detailed ultrasound when cardiomyopathy is suspected. Unlike some recognised forms of HCM, there is no widely used genetic test that screens cats for restrictive cardiomyopathy. In practice, both diseases can lead to the same dangerous endpoints—atrial enlargement, pulmonary congestion and clot formation—so the echocardiographic pattern is important for diagnosis.
Treatment is directed at the complications present in the individual cat. Cats without congestion may require monitoring rather than immediate medication, while those with congestive heart failure usually need diuretics to reduce fluid accumulation. Oxygen and minimal handling are important during an acute breathing crisis.
Marked left atrial enlargement increases the risk of arterial thromboembolism, so antiplatelet medication such as clopidogrel may be prescribed. Additional drugs are selected according to heart rate, blood pressure and the specific echocardiographic findings rather than used automatically in every case.
Home monitoring is important once heart failure has occurred. Sleeping respiratory rate, appetite, activity and medication tolerance can provide early warning of deterioration. Kidney values and hydration are monitored because diuretics that control heart failure can also reduce circulating fluid volume.
There is no procedure that removes the fibrous restriction from the heart muscle. Management therefore aims to control congestion, reduce clot risk and preserve comfort. Prognosis varies, but cats with severe atrial enlargement, recurrent heart failure or thromboembolism generally have more advanced disease. Regular reassessment allows medication to be adjusted before breathing difficulty becomes severe whenever possible.
