Psychogenic alopecia describes excessive grooming and hair removal associated primarily with behavioural stress after medical causes of itch and pain have been excluded. The term should be used cautiously because many cats once labelled “psychogenic” are later found to have allergy, parasites, pain or another physical trigger.
Affected cats often have symmetrical areas of shortened or missing hair on the abdomen, inner thighs, flanks or forelimbs. The skin may look surprisingly normal because the hair has been licked or chewed rather than falling out. Broken hair shafts help distinguish self-induced alopecia from some spontaneous hair-loss disorders.
Stressors can include conflict with other cats, lack of control over resources, major household changes, boredom, unpredictable handling or inability to escape unwanted interaction. Grooming is a normal feline coping behaviour, so repetition can become self-reinforcing even after the original trigger has lessened.
Flea allergy, food allergy, environmental atopy, mites and dermatophytosis must be considered. Pain from arthritis, urinary disease or abdominal discomfort can also cause repeated licking of a particular region.
True psychogenic alopecia is best understood as a welfare and behaviour problem rather than a cosmetic one. The goal is to reduce the cat’s underlying arousal or conflict and provide safer alternative behaviours. Simply preventing grooming with a collar may protect the coat temporarily but does not address why the cat feels driven to groom excessively.
The location of grooming can occasionally point towards pain rather than anxiety. Repeated licking over the lower abdomen may accompany urinary discomfort, while grooming around hips or the lower back can occur with arthritis.
Broken hairs, observed grooming and hair found in vomit or stool can support over-grooming, but they do not identify the cause. Video can help if the cat rarely grooms excessively while people are present.
The first step in assessment is proving that the hair loss is self-induced and looking for medical reasons the cat may be licking.
A complete skin work-up may include flea control, skin cytology, fungal testing and mite investigation. A strict elimination diet can be necessary because food allergy often produces symmetrical grooming without dramatic skin inflammation. The veterinarian also checks for pain, particularly arthritis, bladder disease or abdominal discomfort.
Only after physical causes have been addressed should environmental and behavioural triggers become the main focus. A detailed history looks at relationships between cats, access to food and litter trays, resting places, recent household changes and when grooming occurs.
There is no blood test for psychogenic alopecia. Early recognition means avoiding a premature behavioural label. If medical investigation is incomplete, the cat can spend months being treated for anxiety while an itchy or painful condition continues untreated.
Diagnosis is one of exclusion. Skin sampling and a careful medical work-up are therefore essential before behaviour is assumed. Behavioural interpretation is therefore strongest only after a targeted pain assessment has also been completed.
Management begins by correcting any medical contributors, even when stress also appears important. Flea control, allergy treatment and pain management may reduce grooming substantially before behavioural intervention is needed.
Environmental changes aim to increase predictability and control. Each cat should have access to separate feeding areas, litter trays, water, scratching surfaces, elevated resting places and hiding sites. Conflict may be subtle, so simply observing that cats do not fight openly is not enough. Regular interactive play, food-search activities and consistent routines provide alternative outlets.
When a clear stressor cannot be removed, behavioural modification and pheromone products may be helpful. Some severely affected cats benefit from medication such as an anxiolytic prescribed by a veterinarian familiar with feline behaviour. Medication works best as part of an environmental plan rather than as a substitute for one.
Hair regrowth takes time even after grooming improves. Progress should be measured by reduced licking and improved welfare, not only by coat length. Recurrence should prompt review of both medical and social factors because a previously controlled allergy or pain condition can return and mimic a behavioural relapse.
