Pneumonia is inflammation of the lung tissue and air spaces. In cats it can result from bacterial infection, aspiration of food or vomit, viral disease, fungal infection, parasites or spread from another illness. Because the lungs are responsible for oxygen exchange, pneumonia can become serious quickly when a large area is affected.
Clinical signs include increased respiratory rate, greater effort to breathe, lethargy, fever, poor appetite and sometimes cough or nasal discharge. Cats do not always cough dramatically even with substantial lung disease. Severe cases may breathe with the mouth open, develop blue-tinged gums or become weak from inadequate oxygen.
Aspiration pneumonia deserves particular attention in cats that regurgitate, have megaoesophagus, receive force-feeding incorrectly, or vomit during impaired consciousness. Inhaled stomach contents cause chemical injury and can introduce bacteria into the lungs.
A cat in severe respiratory distress is stabilised before stressful procedures.
Different causes create different patterns of lung injury. Aspiration may be patchy and can worsen after the original event, bacterial infection can spread through connected airways, and fungal or parasitic disease may form a more uneven distribution. Inflammation and inadequate oxygen can then affect circulation, appetite and organ function beyond the chest. Prognosis is influenced by the area involved, speed of progression, oxygenation and underlying disease. Limited feline respiratory reserve means decline can be rapid once compensation fails. Injured lung tissue may remain vulnerable to renewed inflammation or aspiration if the initiating disorder persists after outward breathing improves.
Because cats can compensate quietly until lung reserve is limited, a seriously affected cat may not look dramatic until late in the course. Counting resting breaths and noticing abdominal effort can reveal deterioration earlier than waiting for blue gums or collapse, which represent advanced oxygenation problems.
Open-mouth breathing, cyanosis or collapse is an emergency. A normal absence of cough does not rule out pneumonia. Early detection depends on recognising altered breathing and acting before severe oxygen deprivation develops.
Any cat with persistent increased respiratory rate, abdominal breathing effort, fever, marked lethargy or reduced appetite should be examined promptly.
Chest radiographs are the main first-line imaging test and can show patterns of lung consolidation or inflammation. Repeat radiographs may be needed because early aspiration changes are not always immediately visible. Thoracic ultrasound can also be useful when abnormalities reach the lung surface.
Blood testing assesses hydration, white-cell changes and organ function. When bacterial pneumonia is suspected, airway sampling for cytology and culture can identify the organism and guide antibiotic choice. The cat must be stable enough for sedation or anaesthesia before such procedures. Parasite testing, fungal testing or viral investigation is added according to history.
Cats at risk of aspiration, including those with megaoesophagus or recent regurgitation under sedation, need close respiratory monitoring. There is no routine screening test for healthy cats.
Diagnosis generally uses chest radiographs or other thoracic imaging. Blood tests assess systemic illness, while airway sampling and bacterial culture can guide antibiotic treatment when infection is suspected. Parasite or fungal testing is selected according to exposure and geography.
Treatment starts with stabilising breathing. Cats with low oxygen levels may need oxygen therapy and minimal handling. Intravenous fluids are used carefully because both dehydration and excessive fluid administration can complicate respiratory disease. Pain, fever, nausea and poor appetite are managed as needed.
Bacterial pneumonia requires an appropriate antibiotic course, ideally guided by culture in moderate or severe cases. Treatment often continues beyond obvious clinical improvement, with follow-up examination or imaging used to confirm resolution. Aspiration pneumonia is treated similarly when bacterial infection is involved, while the underlying cause of regurgitation or swallowing dysfunction must also be addressed.
Nebulisation and gentle chest physiotherapy may help mobilise secretions in selected stable cats. Adequate nutrition is important, but feeding methods must avoid further aspiration risk. Severely ill cats may need hospitalisation for several days.
The prognosis is generally better when treatment begins before respiratory failure and when the underlying cause can be corrected. Cats with extensive aspiration, resistant infection or serious concurrent disease have a more guarded outlook. After recovery, any recurrence of rapid breathing, fever or lethargy should be investigated promptly rather than treated empirically at home.
