Pleural effusion is accumulation of abnormal fluid in the space between the lungs and the chest wall. The fluid does not fill the airways; instead, it compresses the lungs from the outside and prevents them from expanding normally. This makes breathing progressively more difficult as the volume increases.
Pleural effusion is a clinical finding rather than one specific disease. Causes include congestive heart failure, feline infectious peritonitis, cancer, bacterial infection of the chest, bleeding, chylothorax and several less common disorders. Identifying the type of fluid provides important clues to the underlying cause.
Affected cats often breathe rapidly and shallowly, sit or crouch with the elbows held away from the body and avoid lying on their side. They may become quiet, stop eating and move as little as possible. Open-mouth breathing or cyanosis indicates severe respiratory compromise. Cats with pleural fluid are easily destabilised by restraint, so handling and diagnostic procedures must be carefully prioritised.
In a distressed cat, removing some fluid with thoracocentesis is both a treatment and a diagnostic step because it immediately gives the lungs more room to expand. The collected fluid can then be analysed for protein, cells, bacteria, triglycerides or other features.
The prognosis depends almost entirely on the cause. A treatable bacterial pyothorax has a very different long-term outlook from advanced cancer or severe heart disease.
The position of the fluid also explains why coughing may be absent. The lungs are compressed externally rather than the airways being filled with mucus, so the dominant sign is often fast, shallow breathing. This is one reason feline respiratory emergencies cannot be classified reliably by whether a cough is present.
Because cats often hide respiratory disease until reserve is limited, a sustained change in resting respiratory rate is more significant than waiting for coughing or collapse.
Rapid, shallow breathing at rest is one of the most useful early warning signs. Owners of cats with known heart disease can monitor sleeping respiratory rate, but any cat with persistent increased effort, reluctance to move or open-mouth breathing needs prompt veterinary assessment.
Auscultation may reveal muffled heart and lung sounds, but imaging confirms the presence of fluid. Point-of-care thoracic ultrasound is particularly useful because it can be performed quickly with minimal restraint in a compromised cat. Chest radiographs may be taken after stabilisation or drainage because severe effusion can obscure underlying structures.
Thoracocentesis allows fluid to be removed and analysed. Cytology, protein measurement, bacterial culture and specialised tests such as triglyceride comparison are selected according to the appearance of the fluid. Echocardiography is important when heart disease is suspected.
There is no general screening test for pleural effusion in healthy cats. Detection depends on recognising altered breathing before the cat is severely distressed.
Thoracic ultrasound can quickly confirm fluid around the lungs. The immediate priority is always to restore breathing safely before pursuing an extensive diagnostic work-up.
A cat struggling to breathe from pleural effusion is stabilised with oxygen and minimal handling. Thoracocentesis is often performed early to remove enough fluid for the lungs to expand more comfortably. This can produce immediate improvement and also provides a sample for diagnosis.
Further treatment is completely cause-dependent. Heart-failure effusion is managed with diuretics and cardiac therapy. Bacterial pyothorax requires antibiotics and often repeated drainage or chest-tube placement. Chylothorax may need dietary, medical or surgical management. FIP, cancer and bleeding disorders each require their own treatment approach.
Fluids can recur, so respiratory rate and effort are monitored closely after drainage. Some cats require repeated thoracocentesis while the underlying disease is brought under control. Chest tubes are used when continuous drainage or lavage is necessary.
The long-term prognosis therefore cannot be predicted from the presence of fluid alone. Owners should understand the specific diagnosis and the likelihood of recurrence. Any renewed rapid breathing, reluctance to lie down, open-mouth breathing or cyanosis is an emergency because a previously drained chest can refill quickly.
Repeated drainage without identifying the cause is not a complete long-term strategy; fluid characterisation and targeted investigation are needed whenever the cat can tolerate them safely.
