Periodontal disease affects the gums, periodontal ligament and bone that support a cat’s teeth. It begins when bacterial plaque accumulates along the gum margin. If plaque is not removed, it can mineralise into calculus, but the major damage comes from inflammation and bacterial activity around and below the gumline rather than from the visible tartar itself.
The earliest stage is gingivitis, where the gums become red, swollen and may bleed. At this point the inflammation can often be reversed if plaque is controlled. With periodontitis, infection and inflammation extend deeper around the tooth. The periodontal ligament and supporting jaw bone are gradually destroyed, pockets form around the roots and teeth may loosen. This loss of support is permanent even when the active disease is later controlled.
Signs can include bad breath, red or bleeding gums, drooling, dropping food, chewing on one side, pawing at the mouth, reduced grooming and weight loss. Many cats continue eating despite significant oral pain, so a normal appetite does not rule out serious disease. Cats may simply swallow food with less chewing or become quieter and less interactive.
Much of periodontal disease is hidden below the gumline.
Periodontal disease is common in adult and older cats, but severity varies. Tooth alignment, oral hygiene, diet, concurrent dental disorders and individual inflammatory responses all influence how quickly damage develops.
As the disease advances, pain can become continuous rather than limited to eating. Chronic inflammation around a diseased tooth also provides a persistent bacterial burden. Treating the mouth therefore improves welfare even when the cat has adapted so well that the owner has not recognised obvious discomfort.
Changes between visits are important, because periodontal disease can progress even when a cat still eats normally. Owners can also lift the lip gently to look for red gum margins or heavy calculus, provided the cat is comfortable with handling.
An oral examination should form part of routine veterinary health checks. The veterinarian looks for plaque, calculus, red or swollen gums, gum recession, fractured teeth, loose teeth and obvious oral pain.
At home, increasing bad breath, drooling, blood around the mouth, a change in chewing, food being dropped, reluctance to eat hard food or reduced grooming all justify a dental assessment.
When disease is suspected, a full dental assessment usually requires general anaesthesia. Dental probing measures pocket depth around individual teeth, and full-mouth dental radiographs are recommended because bone loss, root disease and other painful problems may be invisible above the gumline.
Cats with a history of periodontal disease need regular reassessment because plaque begins to reform soon after cleaning. The interval between professional dental procedures should be based on the individual mouth rather than waiting until severe tartar, loose teeth or obvious pain develops.
A complete assessment cannot be made by looking only at the visible crowns. Examination under general anaesthesia allows each tooth to be probed, cleaned and assessed safely, while dental radiographs show bone loss and root disease that cannot be seen from the surface.
Professional dental treatment removes plaque and calculus from above and below the gumline and allows diseased teeth to be treated properly. Under general anaesthesia the teeth are scaled, polished and probed, and dental radiographs are used to assess roots and supporting bone. Teeth with advanced attachment loss, painful root disease or infection may need extraction.
Antibiotics are not a substitute for dental treatment. They may be appropriate in selected infections or medically compromised cats, but they cannot remove calculus, diseased tooth roots or periodontal pockets. Pain relief is important after extractions and other invasive procedures, and soft food may be recommended for a short period while the mouth heals.
Home care is aimed at slowing the return of plaque. Daily brushing with a cat-safe toothpaste is the most effective option when the cat will tolerate it. Veterinary dental diets, dental treats or oral-care products with recognised plaque-control evidence can provide additional help but do not replace professional assessment.
Follow-up depends on the severity of disease and the cat’s ability to tolerate home care. Advanced periodontitis cannot be reversed, but removing painful, infected teeth and controlling plaque can greatly improve comfort. Many cats eat, groom and interact more normally once chronic oral pain has been treated.
