Otitis externa is inflammation of the external ear canal. In cats it is less commonly a simple recurrent allergy problem than in dogs, so identifying the underlying cause is particularly important. Ear mites, polyps, foreign material, allergy, growths and secondary bacterial or yeast infection can all contribute.
Signs include head shaking, scratching, odour, discharge, redness and pain when the ear is touched. Some cats hold one ear differently or become resistant to head handling. Thick wax, dark debris or pus can accumulate, but appearance alone does not reliably identify the cause.
Chronic inflammation can narrow the canal and may extend deeper towards the middle ear. If the eardrum is damaged, some ear medications become unsafe.
Otitis externa is a description of inflammation, not a final diagnosis. Treating only the secondary infection without addressing mites, polyps, allergy or another primary problem can lead to recurrence. The aim is to restore a comfortable, open ear canal and remove the factor that allowed inflammation to persist.
Ear disease can also be secondary to problems outside the visible canal. A nasopharyngeal polyp, middle-ear infection or mass can drive recurrent discharge and pain despite apparently appropriate drops. Repeated one-sided otitis in a cat is therefore more diagnostically significant than an occasional mild bilateral wax build-up.
A cat with one-sided chronic ear disease, bleeding, a visible mass or persistent pain needs particularly careful investigation. Neurological signs such as head tilt or imbalance increase that concern. Early detection matters because chronic inflammation can thicken and narrow the canal and make treatment more difficult. Regularly inspecting the ears is useful, but unnecessary frequent cleaning of a normal cat ear can itself cause irritation. Recurrent otitis should trigger a search for the underlying cause rather than repeated use of leftover drops.
Any repeated head shaking, ear scratching, odour or discharge should be examined before home treatment.
The veterinarian examines the canal with an otoscope and assesses whether the eardrum can be seen. Cytology of the discharge identifies yeast, cocci, rod-shaped bacteria and inflammatory cells. Ear mites can be sought by microscopy. Culture is reserved for selected infections, especially when rods, treatment failure or resistant bacteria are concerns.
If the canal is too painful or filled with material, sedation may be required for safe cleaning and assessment. CT or other imaging may be recommended when middle-ear disease, a nasopharyngeal polyp or tumour is suspected.
There is no routine screening test beyond ear examination.
Young cats with dark debris are often suspected of having mites, while older cats with one persistently affected ear deserve assessment for a polyp, tumour or other focal obstruction. Diagnosis includes otoscopic examination and cytology of ear discharge. Cytology shows whether bacteria, yeast or inflammatory cells are present and helps guide medication. Mites can be identified microscopically. Imaging or examination under sedation may be needed when the canal is too painful or obstructed to assess properly.
Treatment is selected from the cause identified. Ear mites are treated with an appropriate antiparasitic product. Bacterial or yeast otitis is managed with medication chosen from cytology and, when needed, culture. Anti-inflammatory treatment may be included to reduce swelling and pain.
Cleaning removes wax and debris so medication can contact the canal, but the method and product must be safe for the condition of the eardrum. Painful or obstructed ears may need professional flushing under sedation rather than repeated home cleaning.
A polyp, foreign body or tumour requires removal or other specific treatment. Allergy management is considered when recurrent bilateral inflammation occurs and other causes have been excluded. If infection has extended into the middle ear, treatment is longer and may include systemic medication or surgery.
Follow-up examination is important because an ear can look improved before infection and inflammation have fully resolved. The long-term goal is comfortable ears with minimal unnecessary cleaning and prompt treatment of any primary disease.
The veterinarian may also recommend a recheck after medication is finished to confirm that the canal is no longer inflamed, particularly when the original disease was deep or recurrent.
