Osteoarthritis is progressive deterioration of a synovial joint involving cartilage loss, bone remodelling and chronic inflammation. It is extremely common in older cats and can affect the hips, knees, elbows, spine and other joints. Previous injury, developmental joint disease and excess body weight can accelerate the process, but age-related degeneration also occurs without a known injury.
Cats rarely show the obvious persistent limp that owners expect. Instead, they may jump onto lower surfaces, hesitate before stairs, sleep more, groom the back or hindquarters less effectively or become irritable when touched. Litter-tray accidents can occur when high sides or a difficult route make toileting painful. A previously sociable cat may hide more because movement has become uncomfortable.

Osteoarthritis cannot be reversed, but pain and function can often be improved substantially. The disease should be treated as a chronic pain condition rather than accepted as normal ageing. Cats benefit when owners recognise that reduced activity may be a symptom rather than a personality change.
Long-term care usually combines weight management, environmental modification, appropriate analgesia and activity that maintains muscle without forcing painful movement. The goal is comfortable mobility and participation in normal feline behaviour, not restoring a senior cat to the athletic ability of a kitten.
The disease often progresses slowly enough that households adapt without noticing. A chair may gradually become a stepping stone to the bench, or a second litter tray may be used because stairs are avoided. These small environmental choices can reveal pain long before the cat shows a conventional limp.
Reduced grooming over the lower back, difficulty entering a high-sided litter tray or hesitation before jumping may be early clues. Early detection depends on recognising behavioural signs of pain and mobility loss.
Senior cats should be screened for mobility change during routine health visits. Questions about jumping height, stairs, grooming, play, litter-tray use and willingness to be handled are often more informative than asking whether the cat limps.
Owners can use videos of the cat walking, jumping and using stairs at home. Gradual changes are easier to recognise when compared with older recordings.
The veterinarian palpates joints and the spine and assesses muscle mass and range of motion. Radiographs help identify osteoarthritic change and exclude fractures, tumours or other causes of pain. Blood testing is commonly performed before long-term medication, particularly in older cats with possible kidney disease.
There is no blood screening test that confirms osteoarthritis. Regular reassessment is useful because the condition progresses and a management plan that was adequate one year may no longer provide sufficient comfort.
Radiographs can show joint-space change, new bone formation and remodelling, but x-ray severity does not always match the amount of pain. Diagnosis therefore combines history, examination, observed movement and imaging.
Management is multimodal. Maintaining a lean body condition reduces mechanical load on painful joints. Food, water and litter trays should be easy to reach, and low-entry trays, ramps, steps and non-slip surfaces can reduce daily discomfort. Warm, supportive resting places are often appreciated.
Pain relief is selected according to the cat’s health. Options can include feline-approved non-steroidal anti-inflammatory medication, monoclonal antibody therapy targeting nerve growth factor, and other analgesics depending on region and individual need. Kidney function and other risks are considered when long-term drugs are used.
Regular gentle movement helps preserve muscle. Interactive play can be adapted to keep toys close to the ground, and physiotherapy may benefit selected cats. Forced high-impact exercise is unnecessary and can worsen pain.
Response should be judged by function: jumping, grooming, litter use, social interaction and willingness to move. Owners should not wait for severe lameness before discussing a change in pain control. Osteoarthritis is lifelong, so treatment is adjusted as the cat ages. Good management can restore behaviours that were mistakenly assumed to have disappeared simply because the cat was getting old.
