Oral squamous cell carcinoma is the most common malignant tumour of the feline mouth. It arises from the lining tissues and is usually locally aggressive, invading the jaw, tongue or surrounding oral structures. By the time obvious signs appear, the tumour may already be extensive because cats hide oral pain and continue trying to eat.
Signs can include drooling, blood from the mouth, bad breath, difficulty chewing, dropping food, weight loss and pawing at the face. A tumour under the tongue can interfere with swallowing, while lesions involving the jaw may loosen teeth or cause facial swelling. Some cats are first presented for what appears to be dental disease.
The cause is not fully understood. Environmental exposures and chronic inflammation have been investigated, but no single preventable trigger explains most cases. Unlike cutaneous squamous cell carcinoma on white ears and noses, ultraviolet light is not the main driver of oral disease.
Regional lymph nodes and the chest may also be assessed for spread, although local invasion is often the major treatment challenge.
The prognosis is guarded because complete surgical removal is difficult in many cats and the tumour can severely affect eating and comfort. Early, small lesions offer more treatment options than large masses.
The tumour often spreads across tissue planes before producing a large outward mass. A small ulcer in the mouth can therefore represent more extensive disease beneath the surface.
Repeated apparent dental infection without lasting improvement can conceal an enlarging oral mass beneath inflamed or ulcerated tissue. A lesion should not be assumed to be gingivitis simply because the cat also has dental disease. Delay allows deeper invasion, increasing untreated oral pain and reducing the options for local control.
Persistent drooling, blood-tinged saliva, worsening breath, difficulty eating, weight loss or swelling of the jaw should prompt a full oral examination.
An awake examination may identify an ulcer or growth, but sedation or anaesthesia is often required to inspect beneath the tongue and the back of the mouth properly. Biopsy is essential for diagnosis. Dental radiographs or CT show whether the tumour has invaded bone and help plan surgery or radiation.
The regional lymph nodes are palpated and may be sampled, and chest imaging is considered during staging. Routine blood tests assess general health and anaesthetic suitability but do not diagnose the tumour.
There is no screening blood test for oral squamous cell carcinoma. Early detection depends on routine oral examinations and investigating abnormal tissue promptly.
Diagnosis requires biopsy. Dental radiographs or CT help show invasion into jaw bone and define whether surgery is possible. Any persistent oral ulcer, swelling or unexplained tooth loosening in an older cat should therefore be biopsied rather than treated repeatedly as routine dental inflammation. This is another reason biopsy and imaging are more informative than judging seriousness from how large the visible lesion appears.
Treatment depends on tumour location and extent. Small, accessible lesions may be treated surgically, sometimes requiring removal of part of the jaw. Cats can adapt surprisingly well to selected oral surgeries, but many tumours are too extensive for complete excision by the time they are diagnosed.
Radiation therapy, chemotherapy or other local treatments may be considered, often with palliative rather than curative intent. Veterinary oncology consultation helps clarify which approach is realistic. Pain relief is a major priority and usually needs to be proactive because oral tumours can be extremely uncomfortable.
Nutrition also requires planning. Soft food may be adequate early, while some cats benefit from a feeding tube when chewing or swallowing becomes difficult. Treating secondary infection can improve odour and inflammation but does not control the tumour.
Quality of life should be reassessed frequently. The goals are comfortable eating or assisted nutrition, effective pain control and preservation of normal interaction. Prognosis is generally poor for advanced disease, but earlier lesions may allow more meaningful local treatment. Decisions should be based on comfort and function rather than tumour size alone.
