Megaoesophagus is abnormal dilation and poor motility of the oesophagus, the tube that carries food from the mouth to the stomach. Food and water can remain within the enlarged oesophagus instead of moving normally, then return passively through the mouth. This is regurgitation, which differs from vomiting because there is usually little abdominal effort.
Cats with megaoesophagus may lose weight, eat repeatedly and regurgitate soon after meals. The most serious complication is aspiration pneumonia, which occurs when food, water or saliva enters the lungs. Coughing, fever or increased breathing effort in a regurgitating cat is therefore an emergency concern.
Megaoesophagus can be congenital or acquired. Neuromuscular disorders, oesophageal obstruction, inflammation and some systemic diseases can interfere with motility. In a proportion of cats no underlying cause is identified. Treatable contributors remain important because oesophageal dilation describes impaired transport but does not explain why it developed.
The oesophagus cannot always be restored to normal function. The prognosis depends greatly on the underlying disease and how often pneumonia occurs. A cat that can maintain weight without aspiration can do well despite persistent oesophageal dilation.
The distinction between vomiting and regurgitation is central because it points to different parts of the digestive tract. Vomiting involves active abdominal contractions and originates from the stomach or intestine, while regurgitation reflects failure of oesophageal transport. Mixing the two histories can send the investigation in the wrong direction.
Video of an episode can help because regurgitation is usually passive, with food appearing suddenly and without repeated abdominal contractions. Early detection is important because repeated unrecognised regurgitation can lead to weight loss and aspiration. Prognosis is better when a reversible cause is found and poorer when severe idiopathic dysfunction persists with recurrent pneumonia.
Regurgitation should be distinguished from vomiting whenever possible. Recurrent regurgitation always deserves investigation.
Chest radiographs are the usual first diagnostic test and may show a dilated oesophagus containing gas, fluid or food. They also evaluate the lungs for aspiration pneumonia. Contrast imaging or fluoroscopy can assess movement and identify narrowing, while endoscopy is useful when a foreign body, stricture or mass is possible.
Blood tests and neurological examination look for acquired causes. Specific tests for neuromuscular disease are added when weakness or other signs are present. A kitten with congenital disease and an adult with sudden-onset regurgitation therefore require different investigations.
There is no screening test for healthy cats. Coughing, fever, lethargy or rapid breathing in a cat with known megaoesophagus requires prompt chest assessment.
Chest radiographs often show a gas- or food-filled oesophagus. Contrast studies, fluoroscopy or endoscopy may be used when obstruction or motility needs further evaluation. Blood tests and neurological investigation are selected according to possible causes.
Feeding management is tailored to the individual cat. Small frequent meals are offered in the food consistency that is easiest to swallow. Some cats do better with a slurry, others with formed meatballs. Feeding from an elevated position or keeping the cat upright briefly after eating can use gravity to assist passage into the stomach.
The underlying cause is treated whenever possible. An oesophageal foreign body or stricture may be corrected endoscopically or surgically, while a neuromuscular disorder requires its own therapy. There is no single pro-motility drug that reliably restores normal feline oesophageal function.
Aspiration pneumonia is treated urgently with appropriate antibiotics, oxygen and supportive care when it occurs. Force-feeding a regurgitating cat can increase aspiration risk, so nutrition plans should be designed carefully. Some severe cases require alternative feeding access that bypasses the oesophagus.
Body weight and respiratory signs are monitored closely. The long-term goal is adequate nutrition without repeated aspiration.
Owners should seek immediate veterinary care for fever or increased breathing because aspiration pneumonia may develop even when the previous regurgitation episode seemed minor.
Management is aimed at using gravity and food consistency to move material into the stomach while minimising aspiration.
