Chronic constipation occurs when stool remains in the colon too long, becomes dry and hard and is difficult to pass. Megacolon is a more advanced state in which the colon becomes persistently enlarged and loses much of its normal ability to contract. Once severe dilation is established, constipation becomes increasingly difficult to manage medically.
Cats may strain repeatedly, pass small hard stools, defecate outside the litter tray or go several days without producing stool. Vomiting, reduced appetite, dehydration and lethargy can occur when constipation is severe. Owners sometimes mistake straining for a urinary problem; in male cats especially, inability to urinate must always be excluded because urethral obstruction is an emergency.
Causes include pelvic narrowing after fracture, neurological disease, dehydration, painful arthritis and medications that reduce intestinal motility. Many cases of megacolon are idiopathic, meaning no original cause can be identified. Manx cats can have additional neurological bowel dysfunction.
Pelvic structure and neurological function are evaluated when an underlying cause is suspected.
Early constipation is easier to manage than established megacolon. Repeated cycles of stool retention stretch the colon and further reduce motility. Long-term treatment therefore aims to maintain soft, regular stool before the colon becomes severely dilated and dependent on repeated enemas or surgery.
The colon’s loss of tone can become self-perpetuating. The more it stretches, the less effectively it contracts; the poorer the contraction, the longer stool remains and dries. Breaking that cycle early is the reason regular soft stool is a therapeutic target rather than merely a matter of convenience. Early intervention preserves remaining motility.
For recurrent cases, diet, water intake, activity, medications and litter access are reviewed. Early detection means intervening when constipation is still occasional rather than waiting until the colon is chronically enlarged.
Owners should know the cat’s normal stool frequency and consistency. Repeated straining, very small dry stools, vomiting or no bowel movement for several days warrants assessment. Because urinary obstruction can look similar, a straining male cat should not be assumed to be constipated without confirming urine output.
The veterinarian palpates the abdomen and uses radiographs to determine stool burden, colon diameter and pelvic anatomy. Blood tests assess hydration, kidney function and electrolytes. A rectal or neurological examination may be needed when pelvic narrowing, spinal disease or Manx syndrome is possible.
Older arthritic cats may avoid toileting if a tray is difficult to enter, allowing stool retention to begin for behavioural-looking reasons.
There is no blood screening test for megacolon. Tracking stool frequency is particularly useful in cats with previous episodes because recurrence can be managed sooner.
Diagnosis uses physical examination and abdominal radiographs to assess colon diameter and stool burden. Blood tests look for dehydration, kidney disease and electrolyte abnormalities.
Acute constipation may require rehydration, enemas and, when stool is severely impacted, manual removal under sedation or anaesthesia. Enemas must use cat-safe products; phosphate-containing human enemas can be dangerously toxic to cats.
Long-term management often includes increased water intake, stool-softening medication such as polyethylene glycol or lactulose and a diet selected for the individual cat. Some respond to highly digestible low-residue diets, while others benefit from fibre. Cisapride can improve colonic motility in suitable cases.
Underlying pain, kidney disease or pelvic narrowing should be treated where possible. Regular litter access and low-entry trays help cats with arthritis. Medication is adjusted to produce comfortable formed stool rather than diarrhoea.
When megacolon no longer responds to medical management, subtotal colectomy can provide substantial relief by removing most of the poorly functioning colon. Cats generally adapt well, although stools may be softer initially. Earlier surgical referral is preferable to repeated severe impactions that cause progressive colonic damage. Ongoing monitoring remains important even after control is achieved.
Cats after subtotal colectomy should have hydration and stool quality monitored, but many ultimately pass formed or soft stools without the repeated painful impactions seen before surgery.
